Job DescriptionDirector of Admissions and Case Management Hultquist Place Assisted Living Community Jamestown, NY, US Exempt, Full-Time (40 hours/week) Position Summary: The Director of Admissions and Case Management is responsible for coordinating the resident admission process while providing ongoing case management services that promote residents’ social, emotional, and mental well-being. This position serves as a key liaison between residents, families, referral sources, and internal departments to ensure a seamless transition into care and continued high-quality, person-centered support throughout each resident’s stay.
