    We are seeking a compassionate, organized, and results-driven Intake Coordinator to serve as the primary face and voice of our Social Adult Daycare community. In this role, you will guide seniors and their families through the enrollment process from their very first inquiry. You will be responsible for managing leads, conducting assessments, navigating Managed Long-Term Care (MLTC) insurance or Medicaid eligibility, and ensuring a warm, seamless transition for new participants into our daily programming. Key Responsibilities Lead Management & Enrollment: Serve as the initial point of contact for all inquiries (phone, walk-ins, web leads, and community referrals). Conduct tours of the facility and present our program's value to prospective participants and their families. Intake & Assessment: Complete comprehensive initial intake paperwork, gathering essential social, medical, and functional histories to ensure the program is a safe and enriching fit for the applicant. Insurance & Compliance Navigation: Coordinate closely with Managed Long-Term Care (MLTC) plans, Medicaid representatives, and local social services to verify eligibility, obtain authorizations, and facilitate enrollment approvals. Community Outreach: Build and maintain strong referral relationships with local senior centers, medical offices, hospitals, and community organizations to drive steady enrollment growth. Data & Documentation: Maintain meticulous, HIPAA-compliant electronic records of all prospects, pending enrollments, and active client data. Track conversion metrics and pipeline updates for management review. Onboarding Support: Collaborate with the activities and nursing/care staff to smoothly transition new participants into active daily attendance, ensuring their specific dietary, language, or social preferences are communicated. Qualifications & Skills Experience: 1–3 years of experience in healthcare intake. Experience specifically within an adult daycare, home care agency, or MLTC network is highly preferred. Language Skills: Bi-lingual capabilities in Spanish and English. Communication: Exceptional interpersonal skills. You must be able to discuss sensitive care needs with families experiencing high stress, offering both deep empathy and clear administrative guidance. Administrative Tech: Proficiency with electronic health records (EHR) software and standard office suites (Microsoft Office / Google Workspace). Knowledge Base: Familiarity with local Department of Health regulations, Medicaid eligibility rules, and long-term care insurance navigation. Work Shifts Full time 37.5 hours per week , rotating weekends 11:00PM-7:30AM Duties & Responsibilities Purpose of Position: Under direct supervision of a licensed Nurse or other professional medical staff in a health care setting, performs a variety of clerical and health-related essential duties and responsibilities associated with patient care; performs related work. Examples of Typical Tasks: 1. Checks vital signs, weighs, and measures patients, obtains specimens and records findings on patient’s chart., 2. High school diploma or its educational equivalent, approved by a State’s Department of Education or a recognized accrediting organization; and, 2. Successful completion of training courses including the performance of phlebotomy and electrocardiography (EKG), or completion of a training program or programs that include the performance of phlebotomy and EKG, such as Phlebotomy Technician and EKG Technician training programs, a Patient Care training program, or a Medical Assistant training program; and 1. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).     Mid-Bronx CCRP Head Start Job Description Teacher STATEMENT OF JOB The Teacher coordinates implementation of curriculum that promotes the social, emotional, physical, and cognitive development of Head Start children. Encourage parent involvement in all aspects of the program and their child’s education. To develop individual goals for children transitioning into programs, provide on-going observation and assessment of progress and facilitate transition into kindergarten. QUALIFICATIONS • BA/BS in Early Childhood Education or related field, or MA/MS in Early Childhood Education. • New York State Certification B-2, New York State Initial Certification, or New York State Professional Certification Birth-Grade 2 (Professional Certification mandates 30 hours of Professional Development every 5 years) • Two years teaching experience and/or supervised student teaching., • Physical Requirement: Physical examination (includes screening for tuberculosis) to show that the individual does not pose a risk to the safety and health of others because of communicable diseases. SPECIFIC DUTIES I. Classroom • Plans and implements the educational program for children by using Creative Curriculum and other resources. • Develop and utilize lesson plans, which reflect mandated elements, parental and cultural influences, and promote the social, emotional, physical, and cognitive development of Head Start children. • Individualize one-to-one group activities to reflect the unique needs and strengths of all children in the classroom, including those who are at risk, those with special needs, those who are gifted, and those who are culturally diverse. • Follow a consistent schedule, which includes small and large group experiences, choice time, music and movement, large and small motor activities, language and literacy, problem solving, two or three daily meals, and effective transition between activities. • Encourage experimentation, exploration, cooperation, socialization, ask open-ended questions and listen respectfully to the answers. • Provide an atmosphere that promotes and reinforces parental involvement in the classroom. 2 • Work with other staff and resources in developing specialized planning for children/families as needed, such as speech, occupational, physical, psychological therapist and nutritionist. • Always supervise and monitor children., • Respond to crisis or emergency situations that may occur. Provide first aid or CPR, prevent the spread of blood borne pathogens, and access emergency services as needed. • Accompanies children on trips., • Supervises Assistant Teachers and Volunteers in the classroom., • Participates in case conferences., • Holds parent teacher conferences., • Maintain accurate written records, including assessments, special education documentation, screening instruments, anecdotal observations, family contacts, lesson plans, monthly reports, form 10’s, documents for transition to public school and parent/teacher conference and other required forms. All communications are potentially sensitive and are subject to Head Start’s policy on confidentiality. II. Services • Participate in regularly scheduled team meetings to plan for and deliver collaborative services across all components with educational director and family worker. • Meet regularly with parents and other staff regarding each child’s status and progress. • Conduct required home visits for purposes of assessment and support, and to share information on parenting, educational strategies, and classroom progress. • Plan and conduct regular parent meetings with team members., • Schedule two parent/teacher conferences yearly – Fall and Spring, • Assist in recruitment efforts., • Attend In-Service Training, • Attend meetings, trainings, and appropriate professional development activities. • General maintenance and security of classroom, • Assist in inventory of all site equipment.
