Key Responsibilities * Support medication reconciliation for patients within the designated population. * Review patient profiles to identify discrepancies and potential medication-related issues. * Work closely with pharmacists and healthcare teams to optimize medication regimens. * Collect and analyze medication data, generating reports on adherence and therapeutic outcomes. * Communicate with patients, caregivers, and providers to address medication-related concerns. * Participate in programs that promote medication adherence and preventive care. * Ensure compliance with all regulatory standards, pharmacy laws, and organizational policies. * Contribute to quality improvement initiatives within the pharmacy team. Responsibilities Fabricate, assemble, and install sheet metal products according to specifications. Operate various power tools and hand tools safely and efficiently. Conduct quality checks on finished products to ensure they meet required standards. Utilize forklifts for the movement of materials when necessary.
