The Assistant Geriatric Care Manager provides support to aging and older clients by assisting in the coordination, monitoring, and delivery of culturally sensitive, trauma-informed care and social services. Under the supervision of a licensed or Lead Geriatric Care Manager, this role helps clients maintain independence, dignity, and quality of life while navigating aging-related challenges. The position provides case management, advocacy, home visits, and administrative support to the Adult Services team, with a strong emphasis on language access and cultural competence. JOB RESPONSIBILITIES • Collaborate with Geriatric Care Managers to identify potential resources and support services for clients., • Support implementation and maintenance of individualized care plans for clients., • Coordinate and monitor services such as home care, medical appointments, housing arrangements, and legal/financial resources., • Serve as a liaison between clients, families, healthcare providers, and community resources., • Assist with documentation, case notes, data entry, and reporting in compliance with agency and funder requirements., • Assist with coordination of community events, survivor recognition programs, and group support sessions., • Support program staff with administrative tasks., • Participate in interdisciplinary team meetings and community outreach., • Bachelor's degree in social work, gerontology, nursing, psychology, public health, human services or a related field. Equivalent experience will be considered., • 1–2 years of experience working with older adults, social services, or case management., • Knowledge of aging services, community resources, and public benefit programs., • Commitment to culturally informed practices and a continuous learning environment., • Strong interpersonal, organizational, and communication skills., • Must have reliable transportation. When using a personal vehicle for work-related duties, you are required to provide proof of a valid driver's license, current vehicle registration, and automobile insurance coverage as required by California law, • Experience with trauma-informed, culturally sensitive, client-centered care., • Experience working with immigrants, trauma survivors, or vulnerable populations., • Familiarity with working with the Jewish community., • Knowledge of social services and community partners in Alameda and Contra Costa Counties. This position is a limited term grant funded position with an anticipated end date of June 30, 2026, but may be eligible for an extension based on funding. This position is funded through a long-standing contract that has historically been renewed. While we remain confident in our continued partnerships, we believe in full transparency with prospective team members. The broader landscape of community-based services is currently shifting, with potential changes at state and federal levels that may impact funding structures in the future and our ability to extend this position. COMPENSATION AND BENEFITS The compensation for this position ranges between $30.75-$35.90 based on skills and experience.
