Diagnoses on this unit are primarily cardiac in nature, but the unit also cares for patients with a diverse variety of general medical illnesses. Because of this, our nurses develop a strong set of skills and knowledge that they can use to grow their career throughout the acute care environment. We are piloting a virtual nursing program on this unit, stationing one nurse in a virtual nursing hub during the hours of 0700-1900, while leaving six nurses physically on the unit to provide direct care to patients. The virtual nurse supports the bedside nurse in providing quality care to our patients, and is part of a virtual care model that includes pharmacy and social work. Prior to training and serving in the virtual nurse role, we require 6 months of experience working on the unit. RESPONSIBILITIES: • Assessment. Interview and observe patients to form assessment; document findings, • Relationship Development. Establish a therapeutic alliance with each patient; collaborate with patient and other resources to review objectives and outcomes of care; focus on the patient’s needs and provide a structured process for patient to reach objectives, • Treatment Plan Development. Formulate a comprehensive treatment plan that integrates initial assessment; coordinate care with primary medical providers; continually evaluate and revise treatment plan based on most current assessment, • Caseload Management. Maintain a dynamic caseload and ensure quality care throughout each case REQUIRED QUALIFICATIONS: • Masters Degree in Human Services field required., • LCSW, LMSW, LMHC, LCAT, LMFT or PhD, or permit holder in one of these professions required. Limited permit or license required within 1 year of hire. PREFERRED QUALIFICATIONS: • Two years of mental health experience preferred. PAY RANGE: $63,000.00 - $78,500.00 CITY: Rochester POSTAL CODE: 14608 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
