As a Licensed Vocational Nurse (LVN) at Momentum, you will be an integral part of our team, providing high-quality nursing care and support to individuals with disabilities in our Intermediate Care Facilities (ICF). This role offers you the opportunity to work in a small, close-knit community where you can build meaningful relationships with both the residents we support and your fellow dedicated team members. Licensed Vocational Nurse (LVN) Essential Responsibilities: • Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting, • Implement and oversee the health care plans for residents, • Administer medications and perform medical procedures as needed, • Collaborate with a multi-disciplinary team to ensure the well-being of residents, • Maintain accurate medical records and documentation, • Offer support and guidance to Direct Support Professionals (DSP), • Valid License Vocational Nurse (LVN) license in the state of California, • CPR certification, • Strong communication and interpersonal skills, • A genuine passion for providing quality care and support to individuals with disabilities, • Ability to work effectively in a team-oriented environment A $2,000 Sign-On Bonus* will be paid in two payments: the first payment after 90 days of employment and the second payment after 6 months of employment.
