The Primary Care Physician (PCP) in our organization demonstrates: • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors., • Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients., • Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals. We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCP’s become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers and because PCP’s are leaders in our organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient., • The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient., • It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not)., • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs., • The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office., • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market., • PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company., • Performs other duties as assigned and modified at manager’s discretion. It is expected that PCP’s will make themselves as available to their patients as possible by being open and available for walk-in visits and answering phone calls and messages in a timely manner., • Service Orientation — PCP’s provide care that they would want for a family member or for themselves to each patient at every interaction., • Evidence Based Medicine — The PCP remains updated on evidence-based medicine, but also recognizes that factors outside of traditional medicine, like lifestyle and nutrition, have a large impact on patient health outcomes. The PCP stays up to date on clinical, nutritional, and lifestyle-based interventions to improve outcomes., • Physician Leadership is integral to good healthcare, so the PCP must be willing to continuously work to develop and improve leadership skills for the benefit of one’s patients, their team, their center and the company., • Quality — Our patients deserve the highest quality of care. This requires a willingness to work with the care teams towards achieving high quality outcomes and quality measures. That means physically, mentally, socially and spiritually. Physician wellness is important for sustainability and promoting the health of physicians, staff, and patients., • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company, • Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes., • Fluency in English, verbal and written. The incumbent is responsible for the overall performance of the operational, economical, and people-related center level goals. They lead in providing overall direction to the operation and administrative aspects of the market, utilizing the related resources associated within the Centers. The incumbent will partner closely with the centers’ clinical leadership to ensure patient care, service and clinical metrics, goals and objectives meet and/or exceed expectations. They are a servant leader who provides leadership to multiple teams to ensure growing the business and owning a profit and loss budget at each Center. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • Owns and operates an average annual budget of over $50M for multiple medical centers, staffing up to 150 employees and regularly servicing up to 10,000 patients per year., • Owns and operates the design, development, and implementation of the ongoing strategy for operational excellence and growth within the centers. Ensures all goals are being met/exceeded regarding the overall patient experience. The Lead, Pharmacy Technician is responsible for monitoring and prioritizing the medication dispensing workflow at the market level for all Medication and Pharmacy Technicians. In accordance with state and federal regulations, the incumbent in this role assists the Medication/Pharmacy Techs as well as, physicians and clinicians in the performance of dispensing functions under the Physician Dispensing protocol while adhering to company policies and procedures. He/She facilitates the implementation of policies and procedures and ensures compliance with quality standards. The Lead, Pharmacy Technician ensures that Medication/Pharmacy Technicians have the resources and training necessary to perform their jobs effectively. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • Participates in and assists with implementing processes that improve Medication Adherence in our Centers., • Aids in the implementation of pharmacy objectives as determined by market leadership in partnership with the corporate Medication COE team., • Takes part in the employment cycle of Pharmacy/Medication Techs by participating in the recruitment process, screening of candidates and interviews., • Provides day-to-day oversight of Pharmacy/ Medication Techs led by the Care Supervisors in partnership with the Center Manager., • Trains new and existing personnel on medication dispensing and medication adherence processes., • Meets with Med/Pharm Techs on a weekly basis or as needed to ensure a successful and efficient Medication Adherence/HEDIS Part D program., • Collaborates with the Market Clinical Manager to balance Pharm/Med Tech staffing across the market to ensure adequate coverage., • Reviews medication incident reports and identifies opportunities for improvement to prevent future errors., • Escalates significant issues to the attention of market leadership with recommendations for solutions while maintaining open communication with Medication COE for additional support, tools and resources., • Participates in quality self-assessments. Participates in the development of action plans for identified areas of opportunity., • Meets with Medication COE, HEDIS and Clinical Operations/Quality on a monthly basis or as needed to report feedback., • Performs other duties as assigned and modified at manager’s discretion. The Medical Assistant (Care Promoter 1) is an unlicensed medical professional cross-trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties of Care Promoter 1 vary from setting to setting, depending on the size, location and type. In addition to administrative duties, Care Promoters perform clinical duties under direct physician supervision in accordance with state medical practice acts. Job DescriptionDescription: Clarke Mainstream Services assists anyone involved in mainstreaming students with hearing loss in regular classrooms. Mainstream Itinerant Teachers of the Deaf provide direct and consultative services to students in preschool through high school. Services are provided as stipulated in a student’s IEP and can include consultation with professionals, direct service to students, support to families, program observations and training. Teachers must demonstrate an understanding of, and the ability to communicate, the challenges encountered by students with hearing loss in regular school. Essential Duties/Responsibilities: · Guide schools in the development of an effective support system for students with hearing loss. This includes knowledge of support services, acoustical and audiological supports, speech and language instruction, counseling, and instructional media. · Provide direct service to students, preschool through high school, as indicated in their IEP. These services can be provided as push-in to the classroom or individual pull-out sessions. · Collaborate with teachers as they work to adapt their teaching style to students with hearing loss. Recommend needed modifications and accommodations. · Work in collaboration with reading specialists, SLPs, special education staff and others helping them understand the impact of hearing loss on the student as it applies to their work. · In-service general education staff regarding educating students with hearing loss. · In-service hearing peers regarding the experience of hearing loss and effective communication practices. · Monitor the development of language.
