Forty-one units will be allocated for residents referred by the New York City HIV/AIDS Services Administration (HASA) and twenty-seven apartments will be for adults referred by the New York City Housing Authority (NYCHA). The Case Manager/Substance Abuse Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population. QUALIFICATIONS • Bachelors degree in Public Health, Psychology, Human Services, or a related field preferred., • Associate degree with at least 2 years of experience in health navigation, case management, peer advocacy, or HIV/AIDS services considered., • Experience working with People Living with HIV/AIDS (PLWHA), LGBTQ+ communities, and individuals with mental health/substance use disorders., • Knowledge of harm reduction, trauma-informed care, and motivational interviewing preferred., • Cultural competency in working with diverse populations, including BIPOC, LGBTQ+, and immigrant communities., • Strong interpersonal and excellent verbal and written communication skills., • Ability to work independently and collaboratively in a team setting., • Organizational and problem-solving abilities to assist clients with complex needs., • Strong organizational skills, attention to detail, ability to work in a fast-paced and demanding work environment, • Knowledge of community resources as pertinent to social determinants of health (e.g. housing, food access, transportation, etc.), • Effective client engagement and needs assessment skills, • Experience on NYC health and human services systems, processes, and services, • Competency in Microsoft applications including Word, Excel, and Outlook, • Excellent verbal and written communication skills, • Language Proficiency (English/ Haitian Creole, French and/or Spanish,) preferred., • Must be able to pass a background check, • Passion for HCCs mission.

SERVICE AND CARE COORDINATION • Ensure residents receive quality and appropriate services per organizational principles and program requirements., • Conduct comprehensive biopsychosocial assessment to identify immediate and long term needs in healthcare, food and nutrition, childcare and youth education, employment and vocation, immigration and citizenship status, English language proficiency, trauma care and recovery, and other social need as disclosed by residents., • Coordinate with Case Managers to design service and care plans that correspond to biopsychosocial assessments and address human, health, education and employment, recovery and stability needs., • Coordinate workshops and activities that enhance self-care and healthy habits, cultural and social adjustment, recovery and stabilization, and English language acquisition., • Collaborate with external providers and organizations to assist residents with social and cultural adaptation, address social and human service needs, access preventive and remedial healthcare, and access to mental health services including trauma recovery and therapeutic counseling., • Meet with residents in assigned caseload at least once a week to assess progress with service and care plans. Modify plans as needed., • Coordinate with Case Managers to conduct weekly room inspections to ensure healthy hygiene and housekeeping practices., • Develop and facilitate workshops and activities with external service providers; monitor resident engagement and assess impact of activities and workshops to individual residents and the general resident population., • Document interactions and communications with and on behalf of residents in CARES and AWARDS., • Perform other duties as assigned by supervisor or Program Director., • Communication: Excellent verbal and written communication skills. Position Salary: $20.50 / hour, Part-Time (18hrs bi-weekly) Position Summary Assist the Admissions Worker in implementing enrollment and orientation activities. Assist students in gaining comfort in the use of community resources. Assist Service Coordinators with structuring appointments for off-site visits. Provide counseling supports as needed.
