Job Description Multisystemic Therapist - Emerging Adults POSITION OVERVIEW: · Support emerging adults in their transition from group residential foster care into independent living with a minimum of disruptions and reduce the risk of emerging adults requiring further systemic involvement and placement. · Eliminate or significantly reduce the frequency and intensity of the emerging adult’s referral concerns in mental health, educational/vocational, interpersonal, and behavioral domains. · Empower the emerging adult and their social network with the skills and resources needed to independently address the difficulties that arise in independent living, and to empower the emerging adult to overcome family, peer, school, employment, and community concerns. · The group residence programs served by MST-EA (Cedar Knolls House and Kaplan House) serve transition aged youth displaying a wide range of behaviors including psychiatric diagnoses, trauma and loss, and related family/behavioral issues. The target for MST-EA focuses on transition age youth, 17 to 26 years old, transitioning from the group residence into the community. Program works with the emerging adult from up to 2 months prior to discharge up to 10 months post-discharge to support the successful reintegration back to the community and social network. KEY ESSENTIAL FUNCTIONS: · Conduct mental status exams with clients (as needed). · Conduct MST assessment including review of referral information, identifying and engaging key participants, identifying systemic strengths and weaknesses, and developing an analysis of the fit of problem behaviors within the ecological context. · Engage primary caregiver(s) and/or other key social network members in active change-oriented treatment by identifying and overcoming barriers to engagement. · Develop trauma-informed, strength-based, and culturally-competent treatment plans in collaboration with the client and his/her social network. · Ensure client and social network safety by conducting regular safety assessments and developing comprehensive safety plans. · Be available and on call to provide clinical support to participants and their family of care. · Implement initial case conceptualization, treatment planning, collaborative goal setting, intervention implementation, outcomes review, and strategy revision procedure using the MST Analytic Process. · Facilitate home- and community-based treatment with clients 2-3 times per week per client for four clients. · Maintain clear and concise weekly case summaries that promote peer and supervisory review and feedback, and that demonstrate compliance with the nine MST Principles and the MST Analytic Process. · Ability to engage and collaborate with all relevant systems and key participants within each system, to maintain alignment throughout MST treatment. · Provide direct clinical treatment using methods compatible with MST principles and practices. · Participate in all MST training, supervision, consultation, MST Therapist development activities, and ongoing MST-EA quality assurance system. · Engaging in weekly program team meetings/case conferences/high-risk meetings. · Oversee client progress and ensure proper discharge planning. Principle Responsibilities • Share 24-hour 7-day cell phone coverage with the social worker., • Shares in the pre-placement process with the social worker., • During the first week a child is in placement, the Socio Therapist should visit the foster home at least three times and have daily phone contact to monitor the child’s adjustment to the foster home., • Helps foster parents develop sensitivity and awareness of children’s problems, including the separation and loss experienced by the child., • Establishes and maintains regular contact (a minimum of once per week) with children and foster parents and provides support., • Develops appropriate behavior plans, refining and revising as needed., • Collects and reviews weekly positive time sheets., • Maintain monthly contact with the school, by phone or in person., • Attends CSE and other school meetings with the EFFC program Educational Coordinator., • Maintain weekly telephone contact with each child’s therapist. Document in progress notes., • When a child is in a respite placement for more than a weekend, the Socio Therapist should make at least one home visit and frequent telephone contact to monitor the child’s adjustment to the respite foster home., • For each child who is 14 or older, the Socio Therapist is responsible for working with the child on independent living skills., • Participates in treatment team meetings., • Participates in the recreational activities for the EFFC program. Minimum Requirements Bachelor’s Degree in Social Work, Psychology, or Sociology required. Previous experience preferred. Travel Requirements This position is in Brooklyn, NY with some travel to other boroughs. A Valid NYS Driver’s License if using personal vehicle or agency vehicle to drive on agency business. Job Description About the Role Overall responsibilities include screening, diagnosis, treatment and guidance under the Children & Family Treatment and Support Services Guidelines. These services will enable a greater focus on prevention and early intervention by providing a greater array of available services. The level of service the CFTSS OLP Provider will provide will be dependent upon the ability and criteria needed to provide the service. What You’ll Do • Complete Licensed Evaluations (Assessments) to assess the child/youth’s development, identify behavioral strengths and weaknesses, determine medical necessity to provide/continue services and identify other potential services needed., • Receive referrals and initiate intake process for youth in foster care., • Provide trauma informed, evidence-based individual and group therapy for youths entering pre-placement sites., • Provide Crisis Intervention Activities for child/youth experiencing psychiatric, behavioral, or situational distress., • Develop Treatment Plans describing the child/youth’s conditions, detailing the scope of services to be provided, expected outcomes, frequency, and duration of treatment., • Prepare appropriate and timely written documentation of all clinical services delivered in the Electronic Health Record., • Facilitate team meetings and participate in meetings to discuss the program's needs, • Train other staff in clinically relevant topics and help to create and foster therapeutic milieus Your Qualifications • Master’s degree from accredited program, in Social Work, Psychology, Mental Health Counseling, or related area required., • Must have one of the following NYS (New York State) Professional Licenses OR have a Limited Permit with one of the below degrees:, • Licensed Mental Health Counselor (LMHC), • Licensed Marriage & Family Therapist (LMFT), • Licensed Psychoanalyst, • Licensed Creative Arts Therapist (LCAT), • Licensed Master Social Worker (LMSW), • Licensed Clinical Social Worker (LCSW), • Licensed Psychologist (Ph.D., Psy.D.), • Training and at least 2 years of experience in providing therapy/treatment services for at risk youth and families., • Valid New York State Driver’s License Preferred, • Bilingual/Spanish Preferred Supervisory Responsibility No Work Environment: Clinic/Field/Corporate Office Position Type/Expected Hours of Work: This is a full-time position. You must be available for flexible hours including evenings and possible weekends, shift hours vary depending on the needs of the child and family. Work hours options are 11pm-7pm or 12-8pm. This position is based at designated Rising Ground locations but will require travel to other programs and office locations as well as travelling offsite for meetings as required.
