Job Description BronxWorks helps individuals and families improve their economic and social well-being. From toddlers to seniors, we feed, shelter, teach, and support our neighbors to build a stronger community. BronxWorks has provided homeless outreach in the Bronx via contracts with the NYC Department of Homeless Services since 1995. Our Stebbins Stabilization bed program provides transitional housing and case management services to a caseload of 64 chronically street homeless adults, with the ultimate goal of ending their homelessness. RESPONSIBILITIES • Support the training and supervision of new and/or junior staff members., • Maintain familiarity with social services and resources available both on-site and in the community, and facilitate the sharing of these resources and information amongst staff., • Support scheduling and admin for twice-weekly contracted psychiatric services., • Oversee program functions in a supervisor’s absence, as assigned., • Independently manage a caseload Conduct comprehensive intake assessments., • Work with clients to develop goals and service plans, provide referrals and direct assistance to support clients applying and/or attending appointments for: mental health services, medical care, substance treatment, vital documents, benefits, legal/immigration services, and permanent housing., • Document all interactions with or on behalf of clients; monitor and document client progress toward service plan goals., • Coordinate client care with other service providers; attend regular case conferences with clients and other service providers, as required; support program leadership with concerns and inquiries received from other service providers., • Attend regular supervision sessions with direct supervisor., • Drive agency vehicle to and from client locations and appointments., • Bachelor’s degree in social work or a related field of study., • Four years of related work experience., • Ability to engage and build rapport with populations with high support needs., • Strong oral and written communication skills., • Detail-oriented and resourceful with strong time management and organizational skills., • Proficiency in Microsoft Office suite and other standard business technology., • Bilingual/multilingual preferred, but not required., • Ability to use a computer for prolonged periods., • Ability to occasionally lift and/or move up to 10 pounds., • Ability to stand, walk, or sit for long periods of time., • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork., • Ability to bend and retrieve objects and/or documents., • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation., • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work., • TB Test required within the first 120 days of employment BronxWorks seeks to build an inclusive organization grounded in respect for differences.
