POSITION SUMMARY:Functions as a core member of an integrated behavioral health team that may involve the patients primary care provider in a school-based setting while adhering to all applicable state and federal confidentiality requirements. Works under the direction of licensed behavioral health and/or medical providers, is responsible for coordinating referrals, and supporting behavioral health services as outlined in the patients treatment plan. Position Brief: Campbell County Health is seeking an Advanced Practice Provider (PA or APRN) to join the Department of Geriatrics to support health services in two locations: a rural outpatient practice and in a residential long-term care facility. The ideal candidate will have a strong background and experience in diagnosing and treating a wide range of geriatric illnesses and chronic disease with a focus on comprehensive patient care and quality of life. This full-time position involves working collaboratively with other healthcare professionals within the hospital-based system to provide high-quality medical services, ensuring the best outcomes for our patients.
