Job Description Role and Responsibilities The Waiver Service Coordinator is a licensed profesional who provides primary assistance to waiver applicants/participants in gaining access to needed waiver and Medicaid State Plan services, as well as other local, state, and federally funded educational, vocational, social, and medical services. The Coordinator maintains a maximum caseload of 25 waiver participants. The Service Coordinator assists the applicant in becoming a waiver participant and coordinates and monitors the provision of all services in the service plan once the individual is determined eligible. For individuals transferring from nursing facilities, the Service Coordinator assists the applicant in obtaining and coordinating services that are necessary to return to the community. For those individuals residing in the community, the Service Coordinator facilitates the necessary supports to maintain the individual's health and well-being sufficient to avoid unwanted nursing home placement. Interventions are expected to result in assuring the waiver participant’s health and welfare and increasing independence, integration, and productivity. Essential Functions: • Facilitating the Initial Service Plan (ISP) and waiver program eligibility;, • Coordinating multiple services among multiple providers;, • Securing initial and annual level of care assessments;, • Assuring that Team Meetings are scheduled and held as designated in the service plan, at least every 6-months and provide all waiver providers, participant, and others with written summary of Team meetings and documents accordingly;, • Facilitating the acquisition, oversight, and delivery of service;, • Ensuring annual service plans (Revised Service Plans/RSPs) and required assessments (ex: level of care, LOC) are completed in a timely manner;, • Facilitating the waiver applicant/participant’s Plan of Protective Oversight (PPO) is completed and supports the service plan;, • Conducting monthly face-to-face visits and in-home visits with the participant no less than once a quarter to review the Service Plan (SP), ensure adequate level of satisfaction with services, and ensure progress towards meeting participant’s goals;, • Documenting all visits, contacts and meetings on the required form or within the electronic medical record timely and accurately;, • Maintaining records for at least six (6) years after termination of waiver services;, • Responding to participant crises and emergencies;, • Addressing problems in service provision. Job Description Job Summary: Working from given instructions, will load programs into the controller, review the program and do intermediate setup of machine tools through first piece inspection approval. Essential Duties and Responsibilities: • Accountable for manufacturing complex parts and machine maintenance., • Able to control product quality during production runs using calipers, dial bore gages, micrometers, etc., • Measure and work with tight tolerance in accordance with control plan and blueprint., • Good working knowledge of proper machine operation and safety., • Manufacture product with tight tolerance in accordance with CAD models and blueprints., • May work with prototypes for potential future work., • Must be able to develop and machine basic fixtures with or without prints., • Keeps work area clean and organized., • When requested, may work on special projects and various related assignments., • May be required to edit programs at machine., • May be requested to instruct other personnel on the machine operation., • May be required to work in various areas of operations when necessary. Additionally, Mortgage Consultants must keep up with mortgage loan trends and developments and are well-connected to other financial institutions, lenders, and advisors. In this role, your ultimate goal is to help clients realize and meet their financial goals by responsibly advising them throughout the mortgage loan process, from application to closing. You will be a great fit for our team if you can swiftly identify customer needs, meet loan production goals, and work to ensure customer satisfaction. Primary Functions: • Meet with loan applicants to identify their needs, collecting information for loan applications, and guiding them throughout the entire loan process, • Assists clients with completion of mortgage applications, inspecting completed documents for accuracy and thoroughness, • Review financial information from all relevant parties to determine eligibility, • Submit loan applications in a timely manner, monitoring active loan files on a regular basis, • Prepare thorough and timely pre-qualifications, • Counsel applicants and resolve any loan-related issues that may arise, • Originate and evaluate mortgage loans in compliance with Company guidelines and standards as well as federal, state, and local regulations, • Build relationships with our Real Estate Sales Professionals, including visiting and attending Real Estate Branch meetings on a regular basis, • Outstanding interpersonal skills, • Excellent oral and written communication skills, • Customer service-oriented, • Proficiency in current office technology (Microsoft Office, Google Workspace, etc.), • 1-3 years of experience as a mortgage loan officer required, • High School Diploma or equivalent required, • Thorough knowledge and understanding of all local, state, and federal mortgage loan regulations required, • Prolonged periods sitting at a desk and working on a computer, • Must be able to push, pull, and lift up to 15 lbs.
