Care Team Nurse Practitioner DEPARTMENT: Medical SUPERVISOR: Site Medical Director POSITION: Care Team Nurse Practitioner STATUS: Exempt SUMMARY: The Care Team Nurse Practitioner (CTNP) cares for patients by performing specified visits and supporting the overall function within a care team. The CTNP supports the care team function by caring for patients through responding to medication refills, addressing lab results, completing forms, requesting prior authorizations, and other patient care needs between visits with their primary care provider (PCP). The CTNP cares for patients with acute complaints as identified by the Care Team Registered Nurses and/or who present as walk-ins to the clinic; patients with abnormal lab results who would benefit from an in-person or telehealth visit to explain results; and/or patients with other requests, such as medication refills or form requests, that are best addressed within the context of a visit. DUTIES AND RESPONSIBILITIES Essentials ● At work or staff meeting on time. Communicates with the Site Medical Director if more than 10 minutes late. ● Completes documentation of clinic visits within 24 hours of the patient visit. ● Checks and responds promptly to urgent care-related calls and messages. ● Maintains credentials including license, DEA, and continuing education. ● Demonstrates lifestyle consistent with biblical standards ● Takes ownership of and attains personal patient volume targets.
