The Primary Care Physician (PCP) in our organization demonstrates: • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors., • Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients., • Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals. We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCP’s become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers and because PCP’s are leaders in our organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals. The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient., • The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient., • It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not)., • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs., • The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office., • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market., • PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company., • Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company., • Performs other duties as assigned and modified at manager’s discretion. It is expected that PCP’s will make themselves as available to their patients as possible by being open and available for walk-in visits and answering phone calls and messages in a timely manner., • Service Orientation — PCP’s provide care that they would want for a family member or for themselves to each patient at every interaction., • Evidence Based Medicine — The PCP remains updated on evidence-based medicine, but also recognizes that factors outside of traditional medicine, like lifestyle and nutrition, have a large impact on patient health outcomes. The PCP stays up to date on clinical, nutritional, and lifestyle-based interventions to improve outcomes., • Physician Leadership is integral to good healthcare, so the PCP must be willing to continuously work to develop and improve leadership skills for the benefit of one’s patients, their team, their center and the company., • Quality — Our patients deserve the highest quality of care. This requires a willingness to work with the care teams towards achieving high quality outcomes and quality measures. The Acute Care Nurse will provide warm hand off to the Community Care Nurse when patient is discharged to home and/or from post-acute care facilities. The Acute Care Nurse adheres to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance and policies and procedures.ESSENTIAL JOB DUTIES/RESPONSIBILITIES: • Daily presence of team members at assigned hospitals during core hours as determined by team workflow and that team maintains a balanced caseload., • Detects areas of opportunities regarding proper allocation of healthcare resources in an acute and post-acute setting. Essential Job Functions • The Motor Assistance Vehicle Technician (MAVT/MAVO) is an integral part of the Virgo transportation team. In this position the MAVT/MAVO transports ambulatory patients to and from residences and or skilled nursing facilities to medical treatments and or appointments, and then return them., • Some of the patients are able to ambulate to our vehicles while other must be moved in their wheelchairs, or are able to move on their electric scooters. No person is to ride the lift while standing, or on the electric scooter. The patients are placed in a wheelchair to go up on the lift, and they are moved inside and secured to the vehicle either on the bench seat or in the wheelchair. Should there be an aide or family member accompanying the patient, they must be secured in the vehicle as well., • The MAVT/MAVO must check to see if any belongings or paperwork must accompany the patient for the transport, and then ensure that it gets dropped off with the patient. For each trip the appropriate documentation must be filled out, and where required, obtain the necessary signatures from the patient and /or facility. Responds to member and guest needs, promotes memberships and programs, and maintains cleanliness and organization of the lobby area. Creates a safe and positive atmosphere that promotes member safety and engagement in accordance with YMCA policies and procedures. ESSENTIAL FUNCTIONS: 1. Provides excellent service to members, guests, and program participants in the Y and on the phone, contributing to member retention., 2. Conduct interviews and/or tours responsive to the needs of prospective members; sells memberships., 3. Builds relationships with members; helps members connect with one another and the YMCA., 4. Handles and resolves membership concerns and informs supervisor of unusual situations or unresolved issues., 5. Follows and applies all YMCA policies, rules, regulations and procedures, including emergency and safety procedures dealing with member services. Completes incident and accident reports as necessary., 6. Opens, maintains and closes cash drawer for membership sales in an accurate manner., 7. Enters/updates member information into computer system and maintains member files., 8. May monitor the locker rooms as required., 9. Maintains a professional attitude and appearance at all times., 10. Attends all staff meetings and in-service trainings., 11. Any other duties deemed necessary to ensure the success of the Community YMCA of Eastern Delaware County. QUALIFICATIONS: 1. Minimum age of 16., 2. Certifications: American Heart Association or American Red Cross CPR/AED, and First Aid within 60 days of hire., 3. Complete required trainings., 4. Excellent interpersonal and problem solving skills., 5. Ability to relate effectively to diverse groups of people from all social and economic segments of the community, 6. Basic knowledge of computers. PHYSICAL DEMANDS 1. Able to lift and carry 15-50 lbs., 2. Sufficient strength, agility and mobility to perform essential functions of position., 3. Visual and auditory ability to identify and respond to environmental and other hazards related to the activity., 4. Must be able to see, hear, direct, and assist members, guests, and staff to ensure safety.
