Job Description Lakeview Health | Jacksonville Florida Drug & Alcohol Rehab has helped thousands of people detox, recover and remain abstinent from dangerous substances. We are dual-certified experts in treating alcohol use, opioid use, and substance abuse disorders, as well as the co-occurring conditions that often accompany them. We are seeking a Therapist to add to our dynamic team! Responsibilities • Perform resident assessments to determine level of care within facility, • Develop and implement a plan of treatment and aftercare plans for the resident and/or family, including obtaining collateral information from family and support network, • Evaluate resident progress and revise treatment plan as needed, • Provide knowledgeable of community resources available for residents and families, • Assumes case management responsibilities for assigned residents as needed, • Perform crisis intervention, including management of difficult or resistant residents, utilizing clinically appropriate accepted techniques and methods, • Present resident cases for discussion with the multidisciplinary team and keeps current case presentation outlines for each resident, • Develops, implements, and coordinates educational lectures and therapy programs, including conducting morning focus, community meeting and psych educational lectures, • Responsible for resident, staff, and self-safety at all times, • Takes responsibility for maintaining and monitoring resident records to ensure documentation is completed within program guidelines, • Documentation meets standards, policies, and established timeframes, • Maintains a good working relationship within the department and with other departments, • Treats residents/families with respect and dignity, • Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors, • Facilitates individual, group and family sessions, AMA interventions and individualizes progress notes in a clinically relevant manner, • Sets appropriate professional boundaries and avoid entering into dual relationships, • Stays current with new techniques, advances through seminars, conferences, continuing education classes, professional societies and journals, etc., • Demonstrates skills to be able to intervene in a crisis situation (i.e., Suicide/homicidal ideation or attempts, family conflict, resident to resident conflicts Qualifications • Master’s degree in social work, MHC, Psychology, Public Administration, or similarly related field, • LMHC or LCSW certification or license eligible, • At least two (2) years of experience in a Therapist/Counselor position working directly with residents is preferred, • Must have superior interpersonal skills and be proficient in delivering individual, group, family therapy and case-management services, • Knowledge of federal, state, and local regulations relating to substance abuse programs and knowledge of Joint Commission and DCF requirements is preferred, • Proficient knowledge of Microsoft Office products (Word, Excel, and Outlook) and the ability to navigate EMR systems required, • Excellent documentation skills We’re officially a Great Place To Work®!
