CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The Hotspotting Opioid Program is an evidence-based program in which a care management model will be led by certified recovery peer advocates and clinical professionals with the objective of providing person-centered services to those at risk of opioid overdose in a safe and collaborative way. The program’s focus is to reduce opioid harm, promote a continuum of recovery, and address critical social determinant of health factors. Under the supervision of the Director of Harm Reduction, the individual will be responsible for providing client outreach, interventions for clients, provide crisis consultations, maintain client case records, linkages to care, referral follow up. Case Managers will document interventions according to all funder and agency requirements, submit statistical data and reports, attend weekly supervision, participate and facilitate case conferences, conduct staff trainings on substance abuse topics, and perform other additional tasks as delegated by supervisor. DUTIES & RESPONSIBILITIES: • Perform outreach to potential clients to engage them into services., • Coordinate the implementation of the multidisciplinary treatment plan, with treatment team., • Document all client activities, progress, and clinical intervention in the case record., • Coordinate treatment services with internal programs and external organizations., • Address and remove barriers to enable client access to all necessary components of health care, including mental health and substance use services by coordinating with the treatment team., • Maintain ongoing face-to-face, telephone, and other collateral contacts to support, encourage, and promote the client's commencement and ongoing compliance with the requirements of treatment and/or services outlined by CHASI and the funder, as well as to assist with mitigating barriers to engagement in services, wellness, and recovery, as well as employability by working closely with the treatment team., • Use required systems to track outreach and engagement activity; conduct Health Related Social Needs Assessments (HRSN) assessments using required platforms and tools and make referrals to treatment and recovery services, • Assist clients in coordinating a manageable treatment., • Help to collect documentation for SSI and housing applications as well as provide advocacy services throughout the application process., • Coordinate logistics for care and treatment needs including appointment reminders and consistent follow-up., • Assess plan and treatment adherence and addresses barriers as they arise., • Connect clients to Harm Reduction services including but not limited to naloxone training and kits, and fentanyl test strips., • Reassess client needs and eligibility for the program., • Develop and discuss aftercare plans with clients, referral resources, and other relevant entities., • Performing crisis intervention and problem resolution for clients in need. Coordinating efforts with treatment staff and documenting same in the case record., • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values., • Other duties as assigned. Requirements: QUALIFICATIONS: • At least 1-year of experience providing case management services., • Experience working with individuals managing substance use disorders., • Bachelor’s degree in a related field, CASAC, or CASAC-T preferred., • CRPA Preferred, • Strong knowledge of harm reduction, trauma-informed care, motivational interviewing, and other person-centered intervention models., • Good organizational skills and an orientation toward detail and accuracy., • Bilingual (English/Spanish) preferred., • Computer literate in Microsoft Office applications, email, and ability to navigate the Internet. Job Description POSITION SUMMARY The Health Educator is responsible for the coordination of health education workshops and group nutritional education classes, the review of client social determinant of health assessments, food vouchers, referrals, program eligibility documents, and the tracking and documenting clients’ group attendance and progress in the program. The Health Educator will also ensure accurate data collection and entry into the charts and outcome measurement. ESSENTIAL POSITION FUNCTIONS • Adheres to funder and agency requirements for the provision of Nutrition Health Education and Health Education workshops., • Develops nutrition education and health education workshops that incorporate goals and objectives, frequency and duration of sessions, and facilitate on HIV related health and nutrition education topics., • Work in collaboration with the Case Manager and Intake and Service Coordinator/Program Manager to ensure client services are met and documented on a monthly basis., • Perform and review social determinants of health assessment for food insecurity and adequate food resources and makes appropriate referrals when necessary., • Conduct receipts review of food vouchers and reconciliation of food vouchers with program participants, and documenting progress notes and in reporting database., • Responsible for maintaining client records and works as part of the Continuous Quality Improvement (CQI) and evaluation team for program improvement., • Maintains client records that document group attendance, participation, progress, accomplishments, and additional needs, referrals and goals., • Monitors, in conjunction with the Senior Program Director, the appropriateness of the food resources and service provided to the client., • Establishes connections, communicates frequently with Medical Providers/ Nurse Practitioners, and RDs, and makes referrals as necessary and appropriate., • Review program intake/ screening form, and periodically complete rescreening form to determine reasons for client participation in the program., • Ensure clients’ adherence to all program policies, procedures, and follows program protocols for participation, voucher distribution, food pantry, health workshops, and incentives., • Participate and take lead in program’s community coordination activities, including monthly team meeting held by Senior Director of Health Initiatives., • Communicates and collaborates with Funder and responds timely and appropriately to all requests from funder, Senior Program Director, and Executive Director., • Attend and successfully complete all required training and represent the agency at various venues promoting the program and the agency., • Serves as a liaison between Iris House and program providers and partners., • Demonstrates teamwork, open communication, and has good interpersonal skills., • Participates in in-house and community presentations, research and grant writing., • Performs other related duties as assigned., • This position is fully in-person, and all services are rendered at our West Side Headquarters. POSITION REQUIREMENTS Education: AA/AS or BA/BS in Nutrition Science, Community Health, or related field such as Health Education or Family and Consumer Sciences. Experience: 2-3 years’ experience working in the field of HIV/AIDS or with other chronic illness; 2-3 years’ experience developing and facilitating nutrition education and/or health education. Other Requirements: Bilingual (Spanish/English) strongly preferred; but not required. Must have excellent presentation and writing skills. Program Overview CAMBA’s Resources for Inclusive Support and Empowerment (RISE) Program aims to reduce transmission of HIV, with a focus on men who have sex with men of color (MSMoC), including those who identify as transgender or gender nonconforming (TGNC). RISE will address health promotion through social media campaigns for MSMoC with messages about relevant health issues and preventive health strategies. RISE will also address the social determinant of health (SDOH) social isolation by creating safe, positive spaces for MSMoC to develop meaningful social connections and benefit from the support of peers in overcoming emotional, social, and health issues. The program will conduct support groups for specific LGBTQIA+ populations and host public events open to all LGBTQIA+ individuals in the community. Lastly, RISE will navigate clients to LGBTQIA+-friendly resources in the community with a focus on HIV prevention and legal services that address domestic violence, workplace discrimination or harassment, housing discrimination, and tenancy issues. RISE will provide services to 450 participants. Position: Bilingual (Spanish) Client Advocate Reports To: Assistant Program Manager Location: 1259 Flatbush Ave. What The Client Advocate Does: • Plan, coordinate and facilitate social/peer support events, including group facilitation for clients., • Prepare marketing materials for the program., • Conduct outreach activities at high-risk venues to market the program in order to recruit new clients and reengage former clients., • Develop action plans with clients enrolled in navigation or support services, • Follow-up with clients and with referral organizations regarding client contact and progress with referral organization, • In collaboration with program staff, work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency., • Contact clients via telephone, mail and/or face-to-face home and office visits to ensure adherence to program, and document via progress notes., • Escort clients to appointments (educational, medical, social service, etc.)., • Provide all required information for weekly/monthly/quarterly/annual reports., • Create and maintain client files and make copies of all client documents., • Prescreen clients over the telephone for eligibility and may schedule intake appointments., • Assist clients in completing all CAMBA intake applications and forms., • Schedule appointments for client with referral organizations., • Input client data and client progress information into automated database., • Two (2) years of relevant experience, preferably in a mid-sized (250+ employees) organization., • Good written & verbal communication., • Computer literacy in Microsoft Office Suite, • Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues., • Experience and/or comfort conducting online outreach via social media and dating/ ”hook-up” apps and websites. • Bi-lingual English/Spanish preferred.
