Department/Unit: HBD - Surgery Thoracic Work Shift: Day (United States of America) Salary Range: $83,200.00 - $128,960.00 RN for Cardiothoracic division outpatient office within the department of surgery. Collaborative team effort to provide optimum care to patients requiring scheduled and emergent surgical care. This position supports the thoracic team but also cross covers the cardiac team when required. Under the supervision of the Nursing Supervisor and Nurse Manager, the Ambulatory Care RN provides nursing support to primary and subspecialty providers. This is demonstrated by the use of the nursing process to work collaboratively with the healthcare team in planning, directing, providing, and evaluating patient care. The Ambulatory Care RN is defined within the department to provide patient care, triage, consultation, and referral among team members. Essential Duties and Responsibilities: Generally include the following. Advocacy * Negotiates and secures patient services; coaches the patient in self-advocacy * On the phone and at the office visit, listen to the patient and family goals and concerns. Works with patients, insurance companies, home care agencies, etc. to access providers, community services, medications. * With an initial order of medications, enteral formulas, - verifies if PA needed, or med not covered at all, reviewing other possible options for non-formulary med, clarifying orders, renewing orders Education and Engagement of the patient and family * Assess readiness to learn/learning styles. * Provides patient and family education taking into consideration cultural, religious, language, and motivational, physical, emotional, and cognitive factors into account in the teaching plan * On the phone or in the office instructs about nursing care plans related to results of labs, tests, procedures, medication changes, dietary changes, other changes in interventions. * Provides education regarding interventions and requests, “teach-back” to ensure understanding (in person and via telehealth). * Performs telephone triage including coordination of care and documentation. * Works with physicians and other resources to develop teaching tools for families and patients and/or utilizes educational resources on Intranet. Cross setting communication and transition * Develops the individualized care plan; care planning activities transcend barriers/transitions keeping the patient at the focus. * Nurses utilize evidenced-based practice to plan and implement nursing care plans (interventions and outcomes). * The plan is updated with every phone call, office visit, inpatient admission, etc. * Coordinates and collaborates between specialty and primary providers who develop and share the patient care plan across settings * Care plan transmitted between setting, changes, and updates communicated * Discusses the discharge plan of care for patients that are inpatient and coordinates follow-up interventions needed. * Coordinates admissions with communication regarding the plan of care. * Coordinates referral to the emergency room * Takes calls by MD offices to discuss coordination of care that cannot be addressed by faxing the most recent office visit note. * Works with other departments inside and outside of AMC to coordinate multidisciplinary care * Coordinates home care – setting up for supply deliveries, clarifying verbal orders * Coordinates infusions- insurance, orders, communication to patient, education * Supplies inpatients with educational information as needed * Coordinates referrals to social services departments, WIC * Facilitates the utilization of resources to meet patient outcomes (case management). * Writes letters of medical necessity as indicated * Participates in the patient throughput process in the clinic as indicated and needed (including vital signs, updating medical records, performing diagnostic procedures, assisting the physician and practitioners, facilitating teaching, end of appointment coordination of care) Coaching and counseling of patients and families * Answers questions patients have before and after provider visit. * Patients are directed to bring questions to the visit and/or calls and answers are secured, before, during, and after the visit (in person or via telehealth). * Documents properly, giving age-appropriate care, gathering data correctly (including vital signs), and facilitating care in a customer-friendly manner. * Performs diagnostic tests such as impedance studies, wireless capsule endoscopies Nursing Process * Assesses patient for knowledge understanding diagnosis, needs, treatment, expected outcomes of treatment. * The best evidence is used for interventions/outcomes; the care plan is routinely updated. Major Duties: • Supervise a team of (5) Fair Futures Coaches across various Good Shepherd Services Residential Programs in New York City., • Provide a minimum of weekly supervision to ensure that the educational, employment and vocational needs of participants are being met., • Work closely with The Fair Futures Educational and Permanency Specialists., • Check in with Residential Program leadership on site., • Review documentation in advance (including any progress notes, new academic data, and goals/steps completed over the last week)., • Work collaboratively with multidisciplinary team to implement procedure and process to promote accessibility to career and educational services that would enhance employment opportunities for participants., • Create and maintain community resources to serve as a conduit to career and educational opportunities, including linkages with vocational/educational agencies such as ACS, ACCES-VR, NADAP, One-Stop, DYCD, etc., • Plan and execute fair future events by organizing vocational, employment/educational recruitment and college tours., • Guide Coaches to utilize motivational interviewing to create an individualized Ed/Voc/Career plan to support participants skills, goals, and interest., • Assist Coach with facilitating Preparing Youth for Adulthood (PYA) groups, which focus on skills, such as: healthy communication, employment process, college application process, positive peer relationships, making positive choices, and service learning., • Assist Coach with identifying potential longer-term career pathways based on the participant’s interests, strengths, and academic/vocational plans., • Participate in trainings/ department meetings, Program Evaluation and Planning Dept (PEP) in fulfillment of the program evaluation processes, including documentation/collection, development and monitoring of program data in Connections and outcomes., • Promote and model the seven Sanctuary Model commitments to create a nonviolent and trauma sensitive culture., • Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment., • Ensure Coach has appropriate external support and supplies necessary to effectively execute tasks., • Assist with developing program activities to explore career, educational pathways and workshops to keep participants engaged during the employment search process., • Track and ensure that young people are attaining employment, training, educational and internship opportunities., • Ensure that organizational expectations for placement and retention of young people in jobs, internships, and training are met on a monthly, quarterly and annual basis., • Help Coaches navigate the DOE, academic and career development goal-setting process,, • Facilitate and assist with the submission of financial aid, ETV, ACS Dorm Project/Foster Care Room & Board, & all applicable scholarships including reviewing reference letters and college essays., • Assist participants with writing college essays and personal statements where needed., • Ensure participants are signed up for any entrance interviews/exams and apply to eligible opportunity programs (Upward Bound, etc.); Monitor Pre-SAT/SAT scores of those participants who do take them, and meet with them to discuss/refine college options., • Meet with participants interested in college to have an exploratory and informational session, after they are referred by their Coach, and discuss/develop individualized success plan to complete identified high school or HSE program, • Establish contact with participants in college to ensure they are enrolled in the proper classes each semester, and assist with enrollment, if needed., • Assist young people with creating/updating resume and cover letter as needed., • Facilitate family Team Conferences when needed., • Bachelor’s degree in human services, workforce development, social work, and/or education required with a minimum of five years’ experience within supervisory capacity. ORGANIZATIONAL ROLE: Reports To: Community School Director Supervises: Social Work Interns Schedule: Full-time, Monday – Friday 10:00 AM – 6:00 PM Compensation: $65,000 - $67,000 per year RESPONSIBILITIES: Student and School Support: * Build rapport with students, maintain confidential case files, and conduct one-on-ones * Conduct assessments and develop service plans to address student needs * Integrate trauma-informed and healing-centered practices school-wide * Facilitate access to services through referrals to internal and external supports * Support the creation and implementation of Social Emotional Learning (SEL) strategies and curricula * Implement mental health interventions using a three-tiered support model (universal, targeted, and intensive) Intern Supervision and Team Development: * Oversee and coach a team of social work interns * Conduct regular group supervision and prep bi-weekly intern meetings * Guide interns through documentation, referrals, and case management * Ensure alignment with RTS practice, DOE standards, and ethical social work guidelines Partnerships and Systems Building: * Collaborate with the DOE Office of School Health and School Mental Health Team * Co-create the community school mental health work plan * Build and maintain a robust referral network with community-based providers * Provide culturally responsive training for school staff on SEL and equity practices Compliance and Reporting: * Ensure compliance with all local, state, and federal guidelines * Stay informed of changes in policy, licensing, or best practices * Monitor and report program outcomes as needed * Complete and submit detailed quarterly reports that outline service delivery, student outcomes, referrals, intern contributions, and trends in student need * Use report data to inform program strategy, stakeholder updates, and continuous quality improvement * Collaborate with the Community School Director and site team to ensure data accuracy and timely submission Family & Community Engagement: * Build trusting relationships with families through consistent, culturally responsive communication * Conduct outreach to families regarding student needs, supports, and progress * Facilitate or co-lead parent workshops related to mental health, SEL, or community resources * Collaborate with school and RTS leadership to create a welcoming and inclusive environment for families * Represent the program at school events and afterschool events as needed. * Promote access to support services, including referrals to mental health, housing, or youth employment resources. * Help plan and support community-building events. Data & Continuous Improvement: * Track key metrics tied to student support, attendance, behavior, and engagement * Use data to identify gaps, refine practices, and inform service delivery * Support site team in using data to monitor program effectiveness and student outcomes * Participate in regular review cycles and contribute to school improvement planning QUALIFICATIONS: * A degree in MSW required. LMSW or LCSW preferred from an accredited school of social work. * Minimum 3 years in youth development or early childhood programming * Minimum 4 years of school-based social work experience * At least 1 year of supervisory or intern management experience * Familiarity with SEL and trauma-informed frameworks * Strong communication, critical thinking, and group facilitation skills * Bilingual (Spanish) preferred WORKING CONDITIONS * The position is based in New York City and at times requires travel to multiple program sites. * Must be comfortable working in DOE school buildings and community-based locations. * Ability to work occasional evenings and weekends for events or training. * Must be able to navigate stairs and carry materials up to 25 pounds when needed.
