This position combines client service, healthcare coordination, relationship building, administrative support, and community outreach to ensure families receive exceptional guidance and a seamless experience. \n This is a part-time position (20–25 hours per week) with significant opportunity to grow into a full-time role as our Syracuse office continues to expand. We are seeking someone who is organized, compassionate, proactive, and excited to grow within a collaborative environment. \n Position Details \n\n • Schedule: 20–25 hours per week\n, • Typical Hours: Weekdays between 10:00 a.m. and 6:00 p.m. (flexible scheduling within these hours based on business needs)\n, • Compensation: $22.00–$27.00 per hour, based on experience and qualifications\n, • Bonus opportunity to transition into a full-time Care Transition Advocate position as the business continues to grow.\n\n Primary Responsibilities \n Client Intake & Care Coordination \n\n • Assist with new client intake and onboarding.\n, • Gather client information and ensure records are complete and accurate.\n, • Coordinate the collection of medical records, physician documentation, hospital reports, therapy notes, medication lists, and other clinical information needed to guide care recommendations.\n, • Maintain accurate and timely documentation within the company CRM.\n, • Coordinate communication between clients, families, hospitals, rehabilitation facilities, physician offices, home care agencies, senior living communities, and other healthcare partners.\n, • Schedule consultations, community tours, appointments, and follow-up visits.\n, • Assist Care Transition Advocates in preparing for client meetings and care planning.\n, • Help facilitate smooth, timely care transitions for older adults and their families.\n\n Client & Family Support \n\n • Serve as a compassionate and professional point of contact for clients and family members.\n, • Educate families about the CarePatrol process and available senior care options.\n, • Provide ongoing follow-up throughout the decision-making process.\n, • Ensure every client receives responsive communication and exceptional customer service.\n, • Assist with post-placement follow-up and ongoing client relationship management.\n\n Healthcare Provider Relationships \n\n • Build and maintain professional relationships with hospitals, physician practices, rehabilitation centers, skilled nursing facilities, home health agencies, hospice providers, and other healthcare professionals.\n, • Maintain regular communication with referral sources to support continuity of care.\n, • Deliver educational materials and resources to referral partners.\n, • Support Care Transition Advocates in strengthening existing professional relationships and developing new referral partnerships.\n\n Business Development & Community Outreach \n\n • Support Care Transition Advocates with outside sales activities and referral partner follow-up.\n, • Assist with physician office visits, hospital outreach, networking meetings, and community relationship development.\n, • Help coordinate educational presentations, community events, and outreach initiatives.\n, • Prepare marketing materials and educational resources.\n, • Maintain CRM activity and referral tracking.\n, • Assist with special projects that support business growth and community engagement.\n\n Administrative Support \n\n • Manage incoming phone calls, emails, and client communications.\n, • Maintain organized client records and documentation.\n, • Assist with scheduling, reporting, data entry, and office organization.\n, • Perform additional administrative duties that support the care transition team and overall business operations.\n\n Qualifications \n Preferred qualifications include: \n\n • Associate's or Bachelor's degree in healthcare, social work, communications, business, marketing, or a related field (or equivalent experience).\n, • Experience in healthcare, senior services, care coordination, customer service, community outreach, healthcare marketing, or administrative support preferred.\n, • Experience working with older adults or within the healthcare continuum is highly desirable.\n, • Strong organizational skills with exceptional attention to detail.\n, • Excellent verbal and written communication skills.\n, • Ability to prioritize multiple responsibilities in a fast-paced environment.\n, • Proficiency with Microsoft Office and CRM/database systems.\n, • Professional, compassionate, relationship-focused demeanor.\n, • Valid driver's license and reliable transportation for community visits, healthcare provider meetings, and networking activities.\n\n The Ideal
