Job Description The Clinical Care Management Specialist/Integrated Care Specialist provides clinical recommendations and collaborative support to team members and physical health care staff involved in the Integrated Care Plan (ICP) Program. The Clinical Care Management Specialist reviews clinical information using a holistic care framework, makes recommendations for appropriate levels of care, creates Integrated Care Plans and coordinates services for individuals referred to the ICP team. CFF’s Family Based Services assign a team of one Master level clinician and one Bachelor level case manager to work closely with clients in the home, school and community to address the child’s emotional struggles as well as the larger systemic issues contributing to troublesome behavior. Job Description Position Summary: The Department of Behavioral Health and Intellectual disAbility Services (DBHIDS) and the Philadelphia Fire Department (PFD) Emergency Medical Services Division have established an Alternative Response Unit (ARU) to work in the epicenter of the Opioid epidemic. The ARU will consist of a Coordinator, Paramedic Lieutenant, and a Case Manager, working primarily within the PFD response system. The team will respond to persons who are treated for opioid overdoses who may refuse treatment, family members or any individual who are interested in treatment and require assistance with linkage. The ARU Case Manager will have the knowledge, resources and information of available treatment modalities and connections to social services. This position will support individuals on their road to recovery. The ARU Case Manger will be supervised by the ARU Coordinator and work in conjunction with the team to engage, assess, link and conduct follow-up. Should the individual refuse transfer to an emergency department, the ARU Case Manager and team will engage with the individual and provide information related to accessing drug treatment, harm reduction services (i.e., syringe exchange), overdose education and naloxone distribution. If the individual is amenable to services, the social worker will collect contact information to ensure linkage. When not responding to a call, the ARU staff will be following up with people who were previously resuscitated and refused transport. The case manager, in conjunction with the PFD will be responsible for engaging with community organizations and members. The case manager will also provide overdose education and training to individuals and families. Job DescriptionThe Clinical Care Manager is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Care Manager must utilize clinical knowledge of best practices and evidence based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, Clinical Care Managers consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. Job Description CORA Services is a dynamic and growing not for profit organization in Philadelphia. CORA has a Full-time Licensed Clinician opening in our Clinical Services Division for a mission driven, detail-oriented individual who can provide support to the clients and families served through our LEHB and IBHS programs. The Licensed Clinician will provide therapy services across several programs in the Clinical Services Division including to Philadelphia Law Enforcement Officers, their spouses, and dependents at the LEHB Wellness Center location in Northeast Philadelphia. Therapy services can include individual, couples, and family modalities. This position involves both in-person and virtual counseling and assessment services for the LEHB and Intensive Behavioral Health Services programs. The approximate salary for this position is $67,226 (salary commensurate with experience). In addition, as a Full-time Licensed Clinician your duties will include: • Deliver therapy to clients and families, as defined by the program description, best practice standards and in full compliance with licensure standards., • Prescribe services through a Written Order after completing Levels of Care Assessments., • Develop treatment plans and crisis plans with the child, family, and team that are developmentally appropriate and strengths-based., • Complete accurate and timely clinical documentation, including but not limited to progress notes, treatment plans, assessments and discharge summaries, demonstrating compliance with professional, funder and insurance provider documentation requirements., • Provide crisis assessments and psychological stabilization, as necessary., • Initiate and recommend appropriate referrals for clients as needed., • Adhere to credential and training requirements established by regulatory and licensing entities and funders., • Implement Evidence-Based treatment and service models, some as recommended by funders., • A minimum of 5 years of full-time experience in providing mental health direct services to children, youth or families, • Ability to utilize collaborative, team-oriented approaches to treatment, • Understanding of Evidenced-Based Practices, • Understanding of child and family development and family systems, • Experience with crisis intervention and crisis management, • Knowledge of DSM-5, • Basic knowledge/understanding of common psychotropic medications, • Experience working with diverse populations CORA Services Inc. provides equal employment opportunities (EEO) to all employees and applicants for employment without regard to race, color, religious creed, sex, national origin, age, disability or genetics.
