Job DescriptionOrganizational Summary Maternity Care Coalition (MCC) is a community-based nonprofit organization serving Pennsylvania and Delaware with over four decades of commitment to improving maternal and child health and early care and education. Position Summary The Team Lead supports several aspects of the Healthy Start program, including implementation, coordination, and ongoing monitoring. This position provides direct supervision to two advocates who provide home visiting and case management/care coordination services. As needed, the Team Lead will provide direct services to pregnant and parenting families. Core Functional Responsibilities • Responsible for administrative oversight of case management and care coordination services., • Works closely with the HS Program Manager and Director to monitor program activities and ensure quality of services., • Supports recruiting new hires and supervising two advocates., • Monitors efforts to meet program deliverables and performance measures., • Prepares enrollment and program reports., • Follows quality assurance protocols, regularly reviews client files, monitors data entry, and provides feedback to staff., • Facilitates individual supervisory meetings and regular staff meetings to promote communication, collaboration, and effective execution of goals., • Serves as a liaison between MCC, clients, service providers, and community-based resources., • Plans and coordinates client events, graduations, outreach, and educational content in collaboration with Advocates and the Community Education Specialist., • Conducts client Risk Assessments within assigned zip codes and assigns cases to Advocates., • Provides direct services, including case management, assessments, referrals, and follow-up to a small caseload, as assigned., • Prepares core competency and performance evaluations with input from staff., • Identifies staff training needs, coordinates with the program manager on the development of training plans, and strengthens skills through ongoing professional development., • Completes special projects and other duties as assigned to support departmental priorities. Supporting Responsibilities • Adheres to the MCC’s security guidelines and ensures the appropriate handling of sensitive information., • Attends relevant workshops, trainings, and workgroups to maintain and expand professional knowledge., • Represents MCC and its programs to funders and in the community, including networking events and speaking engagements, as needed. Knowledge, Skills & Abilities • Demonstrated understanding and commitment to MCC’s mission, background, service delivery model, and Theory of Change., • Skilled at building relationships and collaborating with diverse groups, independently and as an effective team member., • Knowledge of concepts and practices related to community health issues, particularly those affecting pregnant women and families with young children., • Demonstrated clinical, social work, or public health skills., • Knowledge of and sensitivity to issues facing the community served., • Strong interpersonal skills demonstrated by compassionate, courteous, cooperative, and professional interactions with diverse staff, partners, clients, and community members., • Proficiency with Microsoft Office Suite and virtual meeting platforms, with the ability to use common software applications, spreadsheets, and customized databases., • Strong project management, documentation, and organizational skills, including the ability to set priorities, meet competing deadlines, and manage time effectively., • Demonstrated understanding of Diversity, Equity, Inclusion, and Justice principles. Model inclusive practices and respect for differences., • Ability to travel across MCC’s service area., • Bilingual (English/Spanish) required. Must be able to communicate fluently in Spanish, required. The assigned work arrangement for this position may include a combination of remote and onsite work and will be determined based on operational needs, role responsibilities, and performance. Work arrangement expectations may be modified over time to meet organizational or programmatic requirements. Staff working remotely are responsible for maintaining a safe and appropriate workspace. • Ability to engage in extended periods of sitting, computer work, and sustained concentration to complete financial analysis, reporting, and related tasks., • Ability to occasionally bend, reach, lift, carry, push, or pull training materials or equipment, generally not exceeding 30 pounds., • Possession of a valid driver’s license issued by the state of residence and the ability to successfully pass a motor vehicle record check, where driving is an essential function of the role., • Ability to travel occasionally to attend conferences, seminars, and professional meetings., • Ability to work non‑traditional hours on an occasional basis based on operational needs., • Employment is contingent upon the successful completion of required post‑offer pre‑employment background clearances, as well as any additional post‑hire background, compliance, or programmatic clearances required by the organization, funders, or applicable regulations. Experience: Three plus years of direct service experience and two plus years of supervisory experience. Education: Bachelor’s degree in social work, public health, or related field.
