Job Summary: The Health Educator is responsible for performing a variety of tasks including curriculum development and implementation, outreach and enrollment services for various programs for high-need families in underserved communities. The Health Educators is an important participant in the continuum of care and will contribute to health promotion and disease prevention by providing support to programs in the Health Education & Outreach Department, connecting individuals and families to appropriate classes, health access, preventive care, oral health services, 0-5 programming, behavioral health support and other allied health services to improve health outcomes. The Health Educator is an integral member of the health education and outreach team and a leader in the community. The health educator works closely with medical providers, TCC staff from various clinics and programs, patients, community members, volunteers and other organizations in the community. This person must be able to communicate effectively and professionally within the health care environment as well as with the residents in the community. The Health Educator Worker must be willing to perform grassroots activities in order to achieve the larger goals and objectives of the Health Education and Outreach Department, as it serves the mission of The Children’s Clinic, “Serving Children and Their Families”. Qualifications: • Bachelor’s degree required, • Experience in and understanding of effective primary prevention, • Experience working with diverse populations and communities with limited access to care, • Bilingual Spanish/English or Khmer/English required, • Experience in coalition work at the local or state level desired, • Excellent speaking, writing and analytical skills, • Ability to provide respectful, empathetic and excellent customer service, • Project coordination experience, • Ability to coordinate multiple projects and deadlines in a high-quality manner with attention to detail, • Ability to work and thrive in a fast-paced, dynamic environment; multi-task and shift priorities as needed, • Strong commitment to achieving meaningful prevention and reducing differences in health outcomes, • Maintain a high level of professionalism and confidentiality, • Proficiency in Windows-based PC environment (word processing, spreadsheet, database, Internet)., • Ability to afternoon hours, evenings and weekends, • A valid California Driver’s License, proof of insurance and access to a car required.Primary Responsibilities/Essential Job Functions:, • Assists in curriculum development and implementation of health education opportunities on various topics including but not limited to: child and family development, chronic conditions, asthma, diabetes, obesity, social determinants of health and leadership, • Strengthens families by using a sensitive, patient-centered approach to build protective factors, • Serves as a trusted liaison for clinic staff, patients and the community, • Fosters positive relationships with community agencies and leaders through outreach and networking and maintains open and effective communication with all TCC departments, • Builds individual and community capacity by increasing knowledge and self-sufficiency through health education, outreach, support in navigating services and other community-based activities, • Represents patients and families in complex interactions involving community agencies, payers and healthcare providers, • Promotes and facilitates health access through education, outreach and enrollment activities as assigned., • Provides direct service, referrals and warm handoffs to a wide variety of resources., • Actively participates in the coordination of groups, classes and health promotion activities, • Establishes trusting relationships with at-risk families by initiating and maintaining regular client contact., • Partners with families and care team to establish goal plans, objectives, and activities based on family's needs., • Maintains an up to date account of quality community resources, provides referrals and warm hand-offs to secure effective linkages to services, • Assist with advancing internal/external opportunities for the Clinic, • Responsible for coordinating assigned projects or grants, • Enters data; maintains accurate documentation in work logs, medical records, calendars, data collection platforms and files, • Submits reports on a timely manner The physical demands of this position are those necessary to successfully perform the essential functions of this job.
