Job DescriptionJob Summary: The Coordinator/QIDP is to support people with intellectual disabilities and special needs to achieve their personal goals. Duties/Responsibilities: This position requires sufficient knowledge and skills to effectively train and monitor Direct Support Professionals' implementation of residents programming. The individual must be able to maintain open communication with all levels of staff and external service vendors. Essential Functions: • Oversees the admissions, evaluation, program development and implementation, program documentation, and discharge., • Serve as a liaison with legal guardians/advocates/families, ombudsman, schools, day programs, state surveyors, and community, • Participate in Louisiana Department of Health and Health Standards, • Oversees the Human Rights, • Participates in weekly briefings with department leaders and managers., • Responsible for coordination, monitoring, documentation, and follow-up on all programs, services, and plans for the individuals on their caseload. Leads and coordinates the interdisciplinary team (IDT) and quarterly meetings., • Monitors and provides oversight of the active treatment plan of all individuals on the assigned caseload., • Interactions with residents in the implementation of individual service plans, education plans or behavior programs., • Coordinate staff training with psychological, speech, occupational and physical therapist consultants and nursing, • Enroll residents into proper day programs and schools and make random visits., • Monitor therapy for programming documentation and report any issues to Program Director and Administrator., • Work with the Social Security Administration to obtain Representative Payee, • Work with Medicaid and Office for Citizens with Developmental Disabilities to obtain residents decision, • Respond to all outstanding and time sensitive mail., • Work with the business office and program managers to ensure residents financial accounts do not exceed $2000., • Must participate in emergency, • Monitor Behavior Support Plans and work closely with the Behavior Specialist to ensure that the least intrusive measures are used, and the Thrive Upstate Behavior Support Policy is followed, • Monitor Residential Plans monthly to ensure plans are being followed and revise training objective accordingly., • Assist with residents appointments if needed. Education and Experience: Must have bachelor's degree in Psychology or Social Work. Master's Degree preferred. Must have at least 1 year of experience working with the intellectual disabled population. Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Must be able to lift fifteen pounds at times. Physical Demands: Sedentary physical activity performing non- strenuous daily activities of an administrative nature. • Stand less than 33% of time on duty., • Walk less than 33% of the time on duty., • Sit between 33% and 66% of time on duty., • Use of hands to finger, handle or feel over, • 66% of time on duty., • Reach with hands or arms less than 33% of time, • Full ability to talk and hear., • Correctable vision required., • Able to lift/carry up to 15 lbs.
