Job Description: We are currently seeking dedicated healthcare professionals to join our team. As a member of Mare Island Home Health, you will have the opportunity to make a profound difference in the lives of our patients and veterans by providing comprehensive care tailored to their unique needs. Positions available include: • Registered Nurse (RN), • Licensed Vocational Nurse (LVN), • Home Health Aide (HHA), • Physical Therapist (PT), • Physical Therapist Assistant (PTA), • Occupational Therapist (OT), • Certified Occupational Therapy Assistant (COTA), • Speech Therapist (ST), • Conduct thorough assessments and develop individualized care plans., • Provide skilled nursing care, therapy services, or social work support according to prescribed treatment plans, • Educate patients and caregivers on disease prevention and management., • Collaborate with interdisciplinary team members to ensure holistic patient care., • Document all patient care services accurately and timely., • Flexible time for home visit, • One on one with your patient, • Independent use of professional skills, • Valid California state licensure or certification in your respective field (RN, LVN, PT, PTA, OT, COTA, ST, MSW)., • Current CPR certification, • Current CA driver's license plus proof of car insurance., • Minimum one year ST clinical experience, (two+ years is more preferable), • Computer/Internet experience (online charting to be used), • Bi/Multi-lingual skills a plus (e.g. Spanish, Russian, Tagalog, etc.), • Competitive compensation package., • Weekly pay, • Flexible scheduling options., • Mileage reimbursement., • Paid sick leave and Individual Retirement Account (IRA) opportunities are available for all employees, • Professional development support.
