Job Description Job Summary This is a full-time professional position. This position is responsible for coordinating referrals and admissions to Transitions LifeCare services in/from targeted facilities. This position will serve as the first line contact for referral processing, admission coordination, and discharge planning from the targeted facilities. This position will be an essential point of contact for developing and maintaining strong relationships with facility staff and leadership. This position will conduct informational visits for all Transitions LifeCare service lines, in addition to admission visits for Transitions HospiceCare, within the targeted facilities and, as needed, in private residences. As well, this position will partner with the Transitions LifeCare Provider Relations team to implement results oriented, innovative education, customer service, account management, and marketing strategies within the targeted facilities to promote appropriate, timely patient referrals and excellence in end-of-life care. These duties will be performed in a manner that is consistent with the agency’s mission statement, core values and regulatory requirements. This position also leads the interdisciplinary team in providing patient centered care to assigned hospice patients. Position is responsible for developing and implementing appropriate care plans and interventions based on the goals of the patient and the interdisciplinary team. Provide innovative education, customer service, account management, and marketing strategies within the provider community (physician practices, hospitals and long-term care communities.) to promote timely, appropriate patient referrals and excellence in end-of-lifecare. The schedule for this position is Monday-Friday 8:00am-5:00pm; MBU On-Call Rotation; 1 major and 1 minor or 3 minor holidays per year. Responsibilities (what you do) • Increase referrals, length of stay and daily census by promoting TL’s Hospice and Palliative Care., • Provide information and resources to increase referral source knowledge and understanding of TL services and benefits to patients at end of life, • Facilitate the referral process, • Offer and coordinate educational in-services as requested by referral sources, • Offer to provide information visits for patients and families interested in agency services, • Follow-up on referrals/admissions within 24-48 business hours with referral source, • Be knowledgeable of patients’ status when visiting referral source, • Acknowledge and thank referral sources in person, phone, e-mail or mail for all referrals, • Acknowledge and report all complaints/concerns by referral source, • Facilitate open communication with facility staff and administration., • Utilize expert level clinical assessment and decision making to implement and manage on-site referral, assessment, and admission processes in targeted facilities., • Provide front-line back-up support to admission team., • Educate about and promote Transitions LifeCare services within targeted facilities to result in an increase in referrals, length of stay, and daily census., • Assist facility staff and leadership in identifying patients appropriate for all Transitions LifeCare services., • Conduct initial screening for incoming referrals from targeted facilities and assist in gathering necessary referral documentation to ensure efficient processing., • Plan, provide, and document safe, competent, and compassionate nursing care to patients and families in facility settings in accordance with physician orders, TL established policies and procedures, and related regulatory guidelines., • Provide informational visits, education, and support to patients/families referred for Transitions LifeCare services in facilities/hospitals, and in private residences upon request., • Conduct hospice admission visits within targeted facilities, including initial assessment, care plan development, and management of immediate needs., • Coordinate and collaborate with IDG team(s) to ensure optimal continuity and excellence in patient care., • Collaborate with other Interdisciplinary Group (IDG) members to determine the patient’s level of care and coordinate the appropriate utilization of contract services., • Coordinate with appropriate facility staff to promote seamless discharges of facility patients to TL community-based services or TL Hospice Home, as appropriate., • Conduct hospice home admission eligibility evaluations upon request at targeted facilities and coordinate with appropriate facility staff to promote seamless patient transfers to hospice home, as appropriate., • Coordinate with patient, family members, physicians, Hospice supervisors and/or clinical staff when requirements of admission are not met/authorized, requiring facilitation of alternative care arrangements, physician consultation, or delay in service., • Make regular rounds at assigned facilities for the purposes of: relationship building, anticipation of referral needs and facilitating proactive responses to the needs of patients who area already active with TL services, • Cross-train to conduct community-based admissions for palliative care and hospice, as needed., • Participate in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings., • Responsible for maintaining current knowledge of best practices and trends in EOL care., • Responsible for providing and documenting appropriate and relevant end of life information and education to patients, families, and community partners., • Participate in the identification of clinical or operational performance improvement opportunities and in performance improvement activities., • Assist in fielding, documenting and addressing client or community partner feedback regarding TL services. Work to resolve any complaints/concerns in a timely manner and communicate resolution to facility administration., • Responsible for professionally and independently managing time, technology, and supplies., • Actively participate in the orientation and training of new employees of all disciplines., • Delegate tasks and supervises the activities of LPNs and Hospice Aides, as needed., • Monitors and manages the appropriate use of patients’ medical equipment, supplies and medications., • Collaborates with other Interdisciplinary Group (IDG) members to determine the patient’s level of care and coordinate the appropriate utilization of contract services., • Participate in the identification of clinical or operational performance improvement opportunities and in performance improvement activities., • Responsible for following current CoPs as related to Certified Nursing Assistant (CNA) supervision and LPN supervision., • Perform other duties as assigned by supervisor. Requirements what you need • Requires expert level analysis of patient clinical information and formulation of an accurate assessment of the patient and his/her condition., • Registered Nurse with current NC license; BSN and CHPN preferred., • Minimum 2 years recent clinical experience required; Hospice, Palliative Care, Home Health, or Skilled Nursing Facility experience or equivalent preferred., • Broad knowledge of hospice, home health, palliative care, and private duty services, reimbursement considerations, and related regulations/standards., • Ability to independently seek out resources and work collaboratively to achieve goals/objectives., • Strong critical thinking, communication and organizational skills required to be effective in this role., • Proven ability to prioritize workflow and multitask with excellent time management skills., • Solid work history demonstrating robust interpersonal skills., • Strong computer skills required; experience with Netsmart, Outlook, and Office 365 preferred., • Must be able to travel to, enter and function in the facility/residence of any patient/family within the service area for providing informational visits or assessment for admission to TL service., • Must have adequate sensory abilities to complete physical assessment, direct patient care, communication with patient/family, care providers and physician, and to complete the follow up documentation and reporting responsibilities associated with referrals processing and admission assessments., • May be required to lift and/or carry items up to 50 pounds., • Current CPR certification., • Work days and scheduled hours with weekend and holiday commitments as outlined in Employee Information Agreement.
