Job Description Position Summary: This position will provide telephone-based technology support to all staff in PMHCC, PMHCC-CTT, and contractual partners. This is a first-level support position for all types of problems or useability questions as it relates to the technologies used within the corporation. Duties and Responsibilities: • Provides telephone technical support for first-level network, computer, printer, peripheral, and software problems, • Creates, updates the status, closes, and reports on Help Desk job tickets for communications received through the Information Services Help Desk, • Performs thorough documentation of all Help Desk tickets from initial assignment to closure, • Creates second-level tickets if an issue requires escalation to the Technical Support Specialists or another area of the Information Services department, • Assists employees who contact the IS Help Desk with software or hardware specific usability questions or issues, • Provides first-level cell phone (iPhone) support, including password resets, IOS updates, wiping devices, email configuration, and joining devices to the corporate Mobile Device Management system (MDM), • Responsible for the creation of Help Desk reports on an as-needed or requested basis, • Meets on a regular basis with the Technical Services Manager and the Technical Support Team on the status of all first-level tickets to ensure all service level commitments are being met, • Responsible for the documentation of all IS procedures relating to processes that fall under this position’s duties and responsibilities, • Works with vendors approved by the Chief Information Officer in handling in-warranty and out-of-warranty repairs of computer equipment, • Keeps current with trends and issues in the technology industry, • Assists the Training & Support Coordinator in managing the software licenses. Job Description The Clinical Care Manager (CCM) is responsible for delivering integrated, person - centered care management to individuals with complex behavioral and physical health needs. The CCM facilitates care coordination across multiple systems, ensures timely utilization management (UM) activities, and promotes collaboration between providers, members, and support systems to improve health outcomes. Clinical Care Managers are assigned to specific teams based on population needs and service settings, with some responsibilities tailored to those contexts. Essential Functions • Deliver longitudinal, integrated care management for assigned members across levels of care and systems., • Apply knowledge of behavioral health systems, service continuums, and medical necessity criteria in all clinical and administrative activities., • Collaborate with members, natural supports, and providers in homes, communities, hospitals, ASAM Rehabs, and virtual environments., • Address social determinants of health and barriers to care using an intergenerational and cross-systems approach., • Ensure Providers are using Evidence-based Practices in treatment planning for members., • Use evidence-based practices to connect members to appropriate behavioral health and physical health treatments. Job Description TITLE: Reunification Support Coach Supervisor DEPARTMENT: DHS/Achieving Reunification Center REPORTS TO: Program Manager PROGRAM OVERVIEW Achieving Reunification Center (ARC), a non-profit entity, has served parents or legal caregivers with children placed in an out of home placement through the Department of Human Services since 2005. The vision of Achieving Reunification Center (ARC) is that all families with children in out-of-home placement have resources and support provided to them to achieve timely reunification. POSITION SUMMARY The Reunification Support Coach Supervisor (RSCS) is responsible for guiding the day to day work of up to five (5) coaches within the RSC Unit. As part of this work, the RSCS is accountable for ensuring that coaches effectively utilize a parent centered coaching model to help parents create goal action plans (GAPS) that directly impact reunification barriers and that the coaches partner with their parents to complete their GAPS. The RSCS conducts regular checks of participant files to monitor GAPs and the delivery of the parent centered coaching model at ARC. Additionally, the RSCS uses data reporting to confirm that parent/caregivers receive high quality support through monthly face-to-face or virtual sessions, as well as follow-ups by telephone and text. The RSCS also uses data reporting to confirm that parents maintain active participation in programming, are connected to needed services and successfully address goals. The RSCS supports coaches to make connections between their work with parents to resolve reunification issues and helping parents to begin the process of strengthening their decision making, persistence and resilience. The RSCS conducts review of reports regarding the progress of parents/caregivers to assigned CUAs and other important parties prior to scheduled Family Court appearances. The role of the RSCS also involves cultivating essential parent centered and motivational interviewing competences and skills through one-on-one supervisory sessions and group meetings. The RSCS also models the ARC organizational values in carrying out the day to day work responsibilities. The RSCS will operate both on-site and remotely. Job DescriptionOrganizational Summary Maternity Care Coalition (MCC) is a community-based nonprofit organization serving Pennsylvania and Delaware with over four decades of commitment to improving maternal and child health and early care and education. MCC’s mission is to improve the health and well-being of pregnant women and parenting families and enhance school readiness for children ages 0-3. We achieve this through direct service, advocacy, and research, in collaboration with individuals, families, providers, and communities. Position Summary The Team Lead supports several aspects of the Healthy Start program, including implementation, coordination, and ongoing monitoring. This position provides direct supervision to two advocates who provide home visiting and case management/care coordination services. As needed, the Team Lead will provide direct services to pregnant and parenting families. Core Functional Responsibilities • Responsible for administrative oversight of case management and care coordination services., • Works closely with the HS Program Manager and Director to monitor program activities and ensure quality of services., • Supports recruiting new hires and supervising two advocates., • Monitors efforts to meet program deliverables and performance measures., • Prepares enrollment and program reports., • Follows quality assurance protocols, regularly reviews client files, monitors data entry, and provides feedback to staff., • Facilitates individual supervisory meetings and regular staff meetings to promote communication, collaboration, and effective execution of goals., • Serves as a liaison between MCC, clients, service providers, and community-based resources., • Plans and coordinates client events, graduations, outreach, and educational content in collaboration with Advocates and the Community Education Specialist., • Conducts client Risk Assessments within assigned zip codes and assigns cases to Advocates., • Provides direct services, including case management, assessments, referrals, and follow-up to a small caseload, as assigned., • Prepares core competency and performance evaluations with input from staff., • Identifies staff training needs, coordinates with the program manager on the development of training plans, and strengthens skills through ongoing professional development., • Completes special projects and other duties as assigned to support departmental priorities. Supporting Responsibilities • Adheres to the MCC’s security guidelines and ensures the appropriate handling of sensitive information., • Attends relevant workshops, trainings, and workgroups to maintain and expand professional knowledge., • Represents MCC and its programs to funders and in the community, including networking events and speaking engagements, as needed. Knowledge, Skills & Abilities • Demonstrated understanding and commitment to MCC’s mission, background, service delivery model, and Theory of Change., • Skilled at building relationships and collaborating with diverse groups, independently and as an effective team member., • Knowledge of concepts and practices related to community health issues, particularly those affecting pregnant women and families with young children., • Demonstrated clinical, social work, or public health skills., • Knowledge of and sensitivity to issues facing the community served., • Strong interpersonal skills demonstrated by compassionate, courteous, cooperative, and professional interactions with diverse staff, partners, clients, and community members., • Proficiency with Microsoft Office Suite and virtual meeting platforms, with the ability to use common software applications, spreadsheets, and customized databases., • Strong project management, documentation, and organizational skills, including the ability to set priorities, meet competing deadlines, and manage time effectively., • Demonstrated understanding of Diversity, Equity, Inclusion, and Justice principles.
