Job Description Licensed Medical Social Worker | Kindful Health Reports to: Director of Clinical Services Position Objective: Are you looking for a position to impact patients and families? Job Summary: Licensed Medical Social Workers (LMSW) of Kindful Health are responsible for the delivery of various social work services to hospice patients and their families. Our Social Workers (LMSW) provide social service counseling and guidance to facilitate the maximum functioning and coping capacity to each patient and caregiver/family member. Essential Education and Experience: Masters of Social Work from an accredited school of social work and holds a current license of Social Work in Texas. Minimum one (1) year experience as a social worker in long term care or medical surgical/acute care setting. One (1) year of hospice experience is preferred but not required. Responsibilities & Duties: The LMSW will be responsible for: • Adhering to all regulatory requirements and professional standards, • Assessing the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicating findings to the RN Case Manager and other members of staff including the interdisciplinary group., • Assessing the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, to include funeral arrangements if needed., • Supervising the implementation of an individualized social work plan of care and notified Bereavement Coordinator of bereavement assessment needs., • Participating in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses and symptoms of the terminal illness., • Providing consultation, education, and support to the interdisciplinary team on social work needs., • Following policies and procedures consistent with recognized standards of professional practice, • Providing high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores Compensation: Salary - $65-75K Annually Benefits - Benefits including health insurance, retirement plan (with matching) and access to other customary benefits and some unique benefits like Virtual MD visits and Roadside Assistance. The Team Manager is the leader of the Patient Care Team and that member of the team whose function is to: supervise, evaluate and coordinate the various component members of the interdisciplinary team; assure continuity of care from admission to discharge or transfer to bereavement; serve as patient advocate and coordinator for other social service and health care providers in the community who are involved in the care of the team’s patients; assume responsibility for the maintenance of patient records from intake through discharge or transfer to bereavement. License to practice in the state where the VITAS program is located.
