Organization Overview: The mission of the Community Health Care Association of New York State (CHCANYS) is to champion community-centered primary care in New York State through leadership, advocacy, and support of Community Health Centers. To do this, CHCANYS serves as the voice of community health centers, working to ensure that every New York State community has primary care that encompasses all aspects of each patient’s health and well-being. CHCANYS is a dynamic, team-oriented, 501(c)(3) non-profit organization, and as New York State’s Primary Care Association (PCA), advocates on behalf of and provides technical assistance to community health centers (CHCs, health centers) throughout New York State. New York’s nearly 900 CHCs sites provide comprehensive, community-based primary and preventative care services to 2.4 million New Yorkers annually, regardless of insurance status or ability to pay. POSITION SUMMARY: This role is part of the Health Center Controlled Network (HCCN) team that works to provide leadership and guidance for data and health technology strategy across the Primary Care Association, the New York State Health Center Controlled Network (HCCN), and the internal workings of CHCANYS. The team also supports all aspects of the Azara Healthcare DRVS platform, known locally as the Center for Primary Care Informatics (CPCI), a statewide data reporting and analytics solution that facilitates clinical practice transformation and quality improvement. Reporting to the Senior Director, Health Center Programs & Services, the Program Manager, Health Center Programs & Services is a critical resource and expert in the use of electronic health record (EHR) systems, health information exchange (HIE) and interoperability, population health management platforms, and other HIT technologies. The Program Manager, Health Center Programs & Services has the primary responsibility of planning, coordinating, and facilitating training and technical assistance (T/TA) around EHR optimization, health IT usability, adoption, implementation and optimization, data management and analytics, value based care, FHIR, artificial intelligence, and interoperability and implementing interventions that help New York State (NYS) community health centers build and maintain proficiency in the use of health IT and digital health tools. The role oversees user groups, committees, work groups, T/TA contracts, keeps informed on the interoperability and health IT landscape (at the federal and state level), creates programmatic documentation and reports, facilitates group learning opportunities, webinars and other HCCN-related or CHCANYS strategic plan-related programming, and supports initiatives that aim to engage EHR vendors and improve health IT usability. This is a hybrid position. (Note-This is not a data analytics or project management role) ESSENTIAL DUTIES AND RESPONSIBILITIES: · Assumes the lead role on assigned projects for health IT, FHIR, interoperability and electronic health record training and technical assistance (T/TA) for health centers, including all aspects of planning, coordination, execution, and evaluation: o Develops programming and T/TA related to HCCN workplan to meet grant deliverables and deadlines and achieves target outcomes and, where applicable, takes part in developing workplan activities o Maintains accurate and up-to-date records in Salesforce CRM and tracks project activities and outcomes; provides status reports on activities/deliverables o Proactively solves problems and follows through on tasks to resolution, escalating to leadership as needed o Develops and facilitates health IT-related webinars and training, including user groups, work groups, and small group learning opportunities to encourage health IT adoption, utilization, and optimization o Develops, facilitates, leads and/or participates in practice transformation activities, committee meetings, annual conference, KLAS Arch Collaborative, and other specialized stakeholder group opportunities to encourage mutually beneficial relationships and communication among health centers, where appropriate o Develops and refines in-house knowledge and expertise around health IT systems, EHR user satisfaction, FHIR, and interoperability as it relates to and is utilized by community health centers (CHCs) o Actively works to develop and expand relationships with health IT vendors and collaborative partners such as other CHCs, PCAs, HCCNs, National Training and Technical Assistance Partners (NTTAPs), subcontractors or subject matter experts o Develops user group content, participates in planning with project partners, as needed, and communicates with health centers at regular intervals to drive knowledge, awareness of health IT vendor services and health center engagement in T/TA activities o Identifies, develops, and implements educational opportunities for health centers and designs activities that help to increase health center staff experience and knowledge on the use of health IT systems, FHIR, interoperability, and health information exchanges (HIEs/RHIOs) · Provides general support and technical assistance as well as individualized and/or small group T/TA to health centers around Health IT to support practice transformation, including but not limited to data management and analytics, interoperability, artificial intelligence, cybersecurity, value-based care, UDS quality measure reporting, FHIR, health information exchange (HIE), patient outreach, and Population Health analytics/tools Minimum Qualifications: · Bachelor’s degree in health sciences, informatics, or related field; Candidates with a master’s degree preferred · Minimum three (3) years of relevant program management experience in EHR adoption/utilization/optimization, quality/performance improvement or reporting, and/or clinical or community health center operations · Proficient in the use of at least one electronic health record (EHR); Registered Health Information Technician (RHIT) certification is a plus · Knowledge of community health, FQHCs, and primary care practice environments required · Knowledge of data analytics, health IT, FHIR, interoperability, and artificial intelligence required · Knowledge of patient-centered care, PCMH, and care models is a plus · Professional experience working in the ambulatory healthcare sector, FQHCs, or community health centers is preferred · Solid relationship building and interpersonal skills, including strong communication, presentation, and training/meeting facilitation skills · Excellent organizational, writing, information gathering, and analytical skills a must · Proven ability to motivate and drive change · Proven ability to thrive in a hybrid environment and demonstrate proactive and innovative approaches to program development · Ability to support health centers, increase engagement, and manage partner relationships remotely · Ability to manage multiple projects simultaneously; high degree of self-management and self-direction • Ability to multitask and manage a heavy workload, organize and coordinate various activities, and work effectively as a team member within and across teams/departments/organization · Comfortable talking with strangers and facilitating online and in-person activities and foster dialog with introverted participants · Ability to work collaboratively with individuals, teams, and populations · Intellectually curious, and a creative problem solver · Open to learning new subject matter, different HCCN-related objective areas · Strong proficiency with communication, Microsoft Office tools including Excel, Word and PowerPoint; Familiarity with Salesforce a plus Must be able to travel throughout the state (within the five boroughs of NYC) to FQHC sites and willingness to represent the HCCN at events and national conferences;
