Clapham South tube station nearby If you are looking to join a supportive and friendly practice, we would love to meet you. • Provide high quality care to our patients, • Conduct face-to-face, telephone and online consultations, • Form part of our total triage team on a pro‑rate basis with the other GPs, • Undertake home visits, • Manage acute and chronic conditions in line with national and local guidelines, • Issue prescriptions appropriately in accordance with practice and NHS policy, • Review test results, hospital letters and correspondence promptly, • Ensure accurate and timely clinical record keeping using EMIS WEB, • Support the training and development of GP registrars, F2s and Allied Health Professionals through clinical supervision Clapham Park Group Practice is a forward thinking, dynamic, training practice set in an attractive leafy part of Clapham, close to the common and Abbeville Village. JOB TITLE: SALARIED GENERAL PRACTITIONER REPORTS TO: THE PARTNERS HOURS OF WORK: TBC LOCATION: 72 CLARENCE AVENUE, LONDON, SW4 8JP Job Description: The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. • Make yourself available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion., • Make professional, autonomous decisions in relation to presenting problems, whether self‑referred or referred from other health care workers within the organisation., • Assess the health care needs of patients with undifferentiated and undiagnosed problems., • Screen patients for disease risk factors and early signs of illness., • Develop care plans for health in consultation with patients and in line with current practice disease management protocols., • Provide counselling and health education., • Admit or discharge patients to and from the caseload and refer to other care providers as appropriate., • Record clear and contemporaneous consultation notes to agreed standards., • Compile and issue computer‑generated acute and repeat prescriptions (avoiding hand‑written prescriptions whenever possible)., • Prescribe in accordance with the practice prescribing formulary (or generically) whenever clinically appropriate., • Undertake all normal duties and responsibilities associated with a GP working within primary care., • Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety., • A commitment to life‑long learning and audit to ensure evidence‑based best practice., • Contribute to evaluation/audit and clinical standard setting within the organisation., • Contribute to the development of computer‑based patient records., • Contribute to the summarising of patient records and read‑coding patient data., • Attend training and events organised by the practice or other agencies, where appropriate., • Respect patient confidentiality and privacy in the course of seeking treatment., • Handle confidential information relating to patients, carers, practice staff and other healthcare workers appropriately., • Ensure confidential information is disclosed only to authorised persons per practice policies and procedures., • Implement and lead on promotion and management of own and others health & safety and infection control as defined in practice policies., • Use personal security systems within the workplace according to guidelines., • Maintain infection control practices, cleanliness and regulatory/contractual/professional requirements., • Provide advice on specimen process, collection, labeling, handling, correct containers, storage and transport arrangements., • Use correct PPE and ensure others do so;
