Essential skills & experience: Strong background in the health & social care sector Experience in completing assessments and care/support planning Excellent communication and IT skills (Microsoft Office, Excel) Organised, self-motivated and adaptable Full UK driving licence and access to your own vehicle Able to provide personal care when necessary and support out in the community Desirable: Previous experience in a domiciliary care coordination role NVQ Level 3 in Health and Social Care (or willingness to work toward it) ??
