As a Senior Care Coordinator at Horizon, you will… • Provide independent, person-centered care coordination for adults living with serious mental illness (SMI)., • Conduct comprehensive biopsychosocial assessments and develop individualized care plans that highlight each member’s needs, goals, and strengths., • Manage an autonomous caseload of approximately 40 members, ensuring high-quality, timely, and consistent care., • Meet with high‑risk members in their homes or community settings to provide intensive, face‑to‑face support., • Connect members with appropriate community resources, benefits, and services to enhance stability and wellbeing., • Collaborate closely with interdisciplinary care teams to ensure coordinated, effective service delivery., • Employ creative problem‑solving to help members overcome practical barriers and achieve their care plan goals. Here are a few specifics of what you’ll need for the job: • Bachelor’s Degree in a Human Services related field required., • Five or more years of experience working with the mental health and substance use population required., • A clean, valid, NYS driver’s license, three years of driving experience, and reliable transportation to travel locally on a regular basis required., • Leadership experience., • Excellent verbal and written communication skills., • Ability to work independently while also being part of a large team. 1000 Young St. Suite 500 Tonawanda, NY 14150 Hours: Monday through Friday - no weekend hours in this position! This position has a schedule from 8:00 AM - 5:00 PM on Monday – Thursday, and on Friday’s 8:00am-2:00pm. Compensation Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $22.95 - $27.00. Starting pay may be impacted based on experience and education. This position includes a bonus earning opportunity, with an annual max potential of $4,800, based on volume of service visits provided and cash equivalent opt out benefits. Compensation adjustments are evaluated annually.
