Job Description Overview: Your assignments may require that you provide services in a family’s home and/or create progress notes that accurately represents the progress that the youth has been making towards the goals that have been completed by other contractors in the treatment plan or behavior support plan. These services provide direct support and assistance for participants, with the caregiver present, in or out of the participant's residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced daily functioning. The assignment ensures that individuals are progressing as they learn and perform the various tasks that are included in basic self-care, social skills, behavioral, and activities of daily living. This basic description may change if you are working with behaviors, etc. Scope of Assignment (may or may not include): • Complete initial intake documentation including but not limited to the intake packet, with signed consent forms, initial notes, etc., • Provide ABA (Applied Behavioral Analysis) therapeutic services within the home, community, and school settings (when applicable)., • Provide ISS (Individual Support Services – Daily Living Skills) therapeutic services within the home, community, and/or school settings (when applicable & within allotted hours)., • Provide IIH/IIC (Intensive In Home Therapy/Intensive In-Community Therapy) services within the home, community, and/or school setting (when applicable & within allotted hours)., • Providing crisis intervention as needed for families., • Collaborate with other team members in order to carry out goals that have been entered in each youth’s treatment plans (Tech II’s, Tech I’s, BT’s, BCBA’s, etc.)., • Create or Implement plans that carry out the successful application of treatment for each individual youth., • Learn new skills by attending ASun Star’s monthly informational sessions., • Obtain and follow new directions on different plans that are updated periodically., • Provide community resources and referrals as appropriate to youth’s needs., • Provide family support as needed and appropriate in supplement to ISS services, • Attend meetings as appropriate and as needed to coordinate care for youth., • Attend 1 hour of clinical monthly group supervision as required by state, • Attend information sessions or elect to attend optional huddles learn best practices in the clinical field. Have at least one year experience in either of the following practices: • Individual Support Services, • Applied Behavior Analysis, • Individual In-Home Therapy, • Individual In-Community Therapy, • Contractors should own and operate an automobile while providing services., • New Jersey State Law Mandates that contractors attend one (1) hour of clinical informational session per month., • The contract requires contractors to complete notes and submit them Every Sunday evening by 11:59PM., • Flexible schedule, • Health insurance Schedule: • Choose your own hours Work Location: In person Company DescriptionFounded in 2005, ASun Star provides services to individuals (children, adolescents, adults) and families within the home, school, and community.
