Be the Best Continuous Care Hospice LVN/LPN You Can Be If you meet these qualifications, we would love to meet you: * Must hold a current Licensed Practical Nurse (LPN) license in the practicing state(s); associate degree preferred. CPR certification is required for direct patient care in Georgia and preferred elsewhere * Requires at least one year of nursing experience in hospice, home health, or a specific patient population (e.g., geriatrics, pediatrics, med-surg) * Strong written, verbal, and organizational skills, along with proficiency in Microsoft Office and EMR systems * Must have a valid driver’s license, reliable vehicle, and meet minimum state-required auto insurance * Able to work as part of an interdisciplinary team with knowledge of relevant state, agency, and federal hospice policies and protocols * Physical demands include frequent lifting (up to 50 lbs), constant assistance with patients (up to 200 lbs), frequent walking, stooping, driving, grasping, and computer use, with occasional exposure to environmental conditions such as temperature changes, noise, chemicals, and uneven walking surfaces Responsibilities: • Deliver skilled nursing care to patients and families in accordance with the interdisciplinary plan of care, including assessment, planning, implementation, and evaluation, • Coordinate with the care team, physicians, and external partners to ensure high-quality, continuous care, while keeping patient records updated and accurately reflecting changes in status, • Educate and support caregivers on medication management, personal care, mobility assistance, end-of-life preparation, and other aspects of patient care, • Serve as a key communicator with the Director of Clinical Operations and interdisciplinary group (IDG) to address any unusual or complex patient or family issues, • Provide compassionate support during critical moments, including at the time of death, and contribute to 24/7 care coverage as needed Caring for others starts with caring for you.
