EOE POSITION SUMMARY: The Program Coordinator is responsible for the providing oversight, coordination and organization of activities appropriate for participants physical, emotional, intellectual and social growth. The Program Coordinator will assist in providing direct support to the Program Director and Assistant Program Director is responsible in assisting in providing quality programming for the needs of the participants that UAU serves within the community. The Program Coordinator is responsible for providing oversight assistance with staff. EOE PRIMARY JOB RESPONSIBILITIES (include but are not limited to): · Provide supervision to both staff and participants to ensure safety and ratio protocols are meet. · Maintain participant files and records and assist with data entry, as directed and required per DYCD · Maintain accurate attendance records on a daily basis as directed and required per DYCD · Assist with participant arrival and dismissal, as needed · Communication with participants and families as required · Model, enforce, teach and develop age appropriate social, physical and emotional behaviors · Lead and/or assist in development and implementation of outreach and deliverable programs to engage potential participants and families within the community. · Create and facilitate a safe, fun and visually appealing productive learning environment for youth · Model, enforce, teach and develop age appropriate social, physical and emotional behaviors · Employ positive behavior management strategies for disruptive and negative behaviors · Demonstrate positive leadership and act as a positive role model to participants · Participate in ongoing professional development · Perform set up and clean up duties every day in alignment with Department of Health regulations and UAU policies · Abide by all safety protocols as outlined by the Department of Health/ OCFS, DYCD and UAU · Assume the responsibilities of the Program Director and or Assistant Program Director in their absence · Performs other related duties as necessary or assigned JOB QUALIFICATIONS: ● Associates Degree preferred, High School diploma or GED (Highschool Equivalency Diploma), required ● Matriculated college student or better from an accredited university or college, preferred ● 1+ year experience working with children and at risk youth, ages 13 to 18 years of age, preferred ● Must have a valid drivers license. ● Must have a commitment to work from a strength-based perspective ● Must be culturally competent - ability to understand, be sensitive to the needs of, communicate with and effectively interact with people across cultures, at risk youth and a variety of age ranges ● Must pass reference checks and background as designated by DOH, DYCD and UAU ● Strong verbal and written communication skills ● Strong problem solving and mathematical skills ● Excellent organizational and time management skills ● Demonstrated ability to conduct and complete work independently ● Willing to take initiative and be flexible when needed ● Excels working in a collaborative environment to achieve target objects and outcomes PHYSICAL REQUIREMENT: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Responsibilities: • Responsible for reading, knowing and complying with all appropriate governing documents to include contracts, agency policies and procedures, State or Federal standards, specific program operations manuals and circulars both internal and external., • Ensures compliance with the agency’s Operations and Personnel Manuals and the Division of Developmental Disabilities Standards Manual., • Completes and submits in a competent, and timely manner all reports, forms, and documentation as required by the agency and Division of Developmental Disabilities., • Understands basic developmental disabilities and learning styles, using motivation techniques to help clients improve in areas with behavioral or medical challenges., • Must complete all CDS trainings and refreshers as directed., • Attend staff meetings, all other relevant meetings and training as directed., • Assist clients with goal outcomes established on the Individual Service Plan (ISP)., • Responsible for reading the ISP and working within the documented expectation for client development., • Provides a consistent means of relaying accurate and complete information to direct supervisors., • Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors according to plans that are in place., • Provides training and supervision to clients in all aspects of community experience living., • Serves as a positive role model for clients and other staff., • Provides safe transportation in agency vehicles for clients as necessary., • Maintains confidentiality and professionalism when communicating with family members or group home staff, Support Coordinators and any outside agency personnel., • Maintains active, positive engagement with clients at all times., • Always ensures the safety and well-being of clients., • Provide direct care and support to individuals with disabilities or special needs., • Proactively accompany and support clients during medical appointments, ensuring relevant information is provided to the healthcare professional. Clearly and accurately communicate details from the appointment to the Residential Coordinator, Medical Compliance Officer, and/or designee., • Follow and demonstrate understanding of medication policy and procedures in the monitoring and administration of the client’s medications. Completes Medication Error Report when needed., • Ensures home, grounds, yard and van are maintained in clean, safe, and sanitary condition., • Ensures meals are prepared and planned based on clients’ individual diet, client input, and the planned menu for the month., • Ensures that the Recreation Calendar is carried out as per client input and as per the planned calendar completed each month., • Responsible for competing all required documentation to include but not limited to, daily logs, communication logs, and behavior reports., • Ensures line of sight for the individual(s) being supported is maintained and completes reports accordingly., • Responsible for adequately and completely cleaning the home during any shift worked., • Ensures that a client’s dignity is not compromised in any way., • Responsible for reporting all emergencies or unusual incidents to direct supervisor., • Cooperates with the licensee and Department staff in any inspection, inquiry or investigation.
