Overview Overview Personal Care Aide (PCA) Staff is responsible for assisting clients with their personal care needs, activities of daily living, home environmental support, and safety, including oversight and supervision for all VNS Health Personal Care clients in accordance with their Scope of Practice as a certified PCA. They work under the supervision of the registered nurse. What You Will Do • Assists clients with personal care activities, including bathing, mouth care, skin care, grooming, toileting, transferring, and ambulation., • Assists clients with housekeeping tasks, including planning and preparing meals, feeding, light housekeeping, changing linens, making beds, and laundering clothes and bedding., • Assists client with safety needs by using equipment (side rails, foot stools, wheelchairs, etc.)., • Assist clients with transportation to appointments, errands, and social activities., • Reminds client to take their medications at prescribed times., • Follows the Plan of Care as indicated., • Provides emotional support and companionship to clients, engaging them in conversation and activities., • Establishes and maintains effective communication regarding the client’s care with the client, family, and supervising healthcare professionals., • Maintains accurate records of care provided, including activities, observations, and incidents., • Completes all VNS Health Personal Care’s records and reports according to agency policy and procedure., • Accurately documents services provided by VNS Health Personal Care policy on the day services are rendered., • Communicates and works cooperatively with other members of the healthcare team Qualifications Qualifications • Satisfactory completion of New York State Health Department approved PCA Training Program or Equivalency., • Sixth (6) grade reading comprehension level., • Writing and math skill level to the extent necessary for the successful and safe performance of required job functions., • Must be at least eighteen (18) years of age. Position Summary As a Home Health Speech-Language Pathologist, you will evaluate and treat patients with speech, language, cognitive, communication, voice, feeding, and swallowing disorders. You'll collaborate with patients, families, physicians, and an interdisciplinary care team to develop individualized treatment plans that promote independence, improve functional outcomes, and enhance quality of life. For pediatric patients, you'll support developmental milestones and communication skills through family-centered, in-home therapy. For adult patients, you'll focus on restoring function following illness, injury, or neurological conditions. What You'll DoConduct Comprehensive Evaluations Assess speech, language, cognitive, voice, fluency, communication, feeding, and swallowing disorders. Evaluate developmental delays in pediatric patients and functional deficits in adult populations. Develop Individualized Treatment Plans Create evidence-based treatment goals tailored to each patient's age, diagnosis, and functional needs. Modify treatment plans based on patient progress and physician recommendations. Deliver Skilled Home-Based Therapy Provide one-on-one therapy in patients' homes. Help patients improve communication, cognitive abilities, swallowing safety, and independence with daily activities. Educate Patients & Caregivers Train families and caregivers on home exercise programs, communication strategies, feeding techniques, adaptive equipment, and environmental modifications. Collaborate with the Care Team Coordinate care with physicians, nurses, PTs, OTs, case managers, social workers, schools, and other healthcare professionals. Maintain Clinical Documentation Complete evaluations, treatment notes, progress reports, and required documentation accurately and on time.
