The PATH team provides behavioral health services to single adults and couples who are actively experiencing homelessness or are at-risk of homelessness and have a severe and persistent mental illness. The Behavioral Health Clinician I provides therapeutic interventions, assessments and related diagnoses, consultation, education, and referral/linkage for individuals with behavioral health concerns in accordance with the goals of the PATH program and the overall mission, values, and objectives of CCH.
