Job Description POSITION SUMMARY: The Health Resources Coordinator (HRC) is responsible for upholding the Health Center’s mission by assuring that all clients receive high quality, comprehensive health education within city high schools. This position includes but is not limited to: providing age and developmentally appropriate health education to students; engage with students to promote increased knowledge and positive health outcomes; and providers linkages to care. Health Resource Center Program in Southeastern Pennsylvania is currently administered by AccessMatters in collaboration with the School District of Philadelphia, local healthcare providers, the Philadelphia Department of Public Health’s AIDS Activities Coordinating Office (AACO), and the Pennsylvania Department of Health (PADOH). ESSENTIAL DUTIES & RESPONSIBILITIES: • Manage the day-to-day operations of the HRC 40 hours per week divided between two schools, • Provide RESPECT sessions to all students who qualify for the evidenced-based intervention, • Provide comprehensive, high quality, evidence-based sexual health education and counseling in the areas of voluntary family planning, sexually transmitted diseases, HIV/AIDS, male/female anatomy, , , and healthy relationships, etc., • Promote positive attitudes, behavior change and risk reduction techniques, including increasing students’ self-efficacy and healthy decision making, reducing STIs, and increasing knowledge and skills, etc. for clients served., • Refer clients to HRC+ Family Planning Providers on site or to FPCSN health centers for primary care, dental and/or behavioral health services as appropriate., • Maintain adequate supplies and provide safer sex supplies to eligible students., • Develop and implement a marketing plan for the HRC to promote and increase access to all students who seek out the HRC., • Identify barriers to access the HRC and problem-solve in collaboration with FPCSN, school contacts, and Access Matters to remove/reduce barriers to access., • Complete all required data entry and reports necessary., • Participate in the HRC monthly meetings and in-service trainings, as required., • Develop and participate in health education and outreach activities, • Provide support for school-based health classes., • Maintain an environment that promotes and supports confidentiality., • Implement and maintain quality improvement activities related to the HRC, • Maintain current Access Matters and FPCSN policies and guidelines., • Performs other duties and responsibilities as assigned. EXPERIENCE AND REQUIREMENTS: Candidates should possess a diverse range of skills and experience including but not limited to: Required Education and Experience • 1 – 3 years’ experience working with adolescents and reproductive health highly preferred, • Excellent organizational skills to manage new project, • Thorough knowledge on and comfortable with health, family planning methods, disease prevention, , and the ability to relay information to students in an age-appropriate and culturally sensitive manner., • Excellent interpersonal, customer service and problem-solving skills., • Excellent group leadership and facilitation skills., • Proficiency in computer skills and willingness to learn all computer systems., • Unconditional ability to maintain HIPAA/confidentiality regulations., • Leadership and high initiative skills that include the ability and desire to champion the HRC program, • Self-motivated, with ability to work with little direct supervision., • Commitment to working with youth in a respectful and empowering way., • Ability to pass required background screenings and credentialing verifications., • Required Immunizations: Hep B Vaccination and TB/PPD Test., • Optional Immunizations: COVID Vaccination and Flu Vaccination., • Current valid CPR Certification required., • Maintain current valid applicable position credentials., • Maintain reliable transportation to travel throughout the network and/or participating events outside the network as needed. LIGHT – Exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.
