The Role As a Tech Lead / Principal Software Engineer on the DeFi Yield Gen Platform team, you’ll get the opportunity to develop your skills and collaborate with engineers across Fidelity in a fun and dynamic environment. You will guide technical direction and work closely with Product Delivery and Architects to build a secure crypto yield generation platform utilizing modern software development practices. Minimum Qualifications: Education and Experience: • Graduate of an accredited nursing program required; Associates Degree in Nursing (ADN), Bachelor of Science in Nurse (BSN) and/or Master of Nursing (MSN), • Current state licensure as a Registered Nurse, • Current CPR certification (AHA BLS), • To maintain on-call status on an intermittent or regularly scheduled basis., • Ability to satisfactorily communicate in English including reading, writing, speaking, and understanding, • In conjunction with the Vice President, ensures dialysis unit operates in compliance with all ESRD, HCFA, federal, AAMI, OSHA and company policies and procedures, • Responsible for training and development of Skilled Nursing Facility staff while providing clinical oversight to non-licensed support team delegating tasks as appropriate, • Knows and follows policies and procedures under CMS ESRD and state regulations and Rendevor Dialysis, monitors staff compliance of same, • Monitors and assures patients receive adequate medical attention during dialysis treatments and pertaining to dialysis care while a patient of the Skilled Nursing Facility in the dialysis unit, • Conducts comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient, identify gaps in care and barriers to good health, • Based on this assessment, and in conjunction with the patient care team, create and implement a care plan that will address identified needs, remove barriers to care, and improve the health of the patient, • Coordinates care by serving as the advocate and resource for the patient, their family, and their provider(s), • Facilitates continuity of care, spanning settings such as the home, hospital, skilled nursing facility, and acute care facility, • Manages patients during periods of transitions of care minimize avoidable readmissions, • Assesses the patient's knowledge of their renal condition and provide education and self-management support, • Provides ongoing reassessment and follow-up to improve patient outcomes, • Reviews and maintains updated policies and procedures on a regular basis, • Develops and monitors quality control measures to assure continuous quality of patient care, • Facilitates Quality Assurance Meetings, • Participates in the IDT and QAPI committee and attend meetings as needed and/or requested, • Identifies and reports concerns, issues, and opportunities for improvement when identified, • Participates in performance improvement projects within licensure, certification, and scope of practice, • Participates in annual QAPI training and other continuing education, as assigned, and required, • Provides suggestions for alternate strategies for indicators not improving, • Assists in building and maintaining a facility/clinic culture of safety and quality, • Reports errors and near misses so that steps can be taken to prevent recurrence, • Reports any negative feedback, complaints, grievances from other staff members, patients, and families, • Uses evidence-based guidelines and proper infection control practices, • Maintains open communication with Skilled Nursing Facility Administrator, Medical Director and Rendevor Dialysis management and staff, • Utilizes resources to improve skills, knowledge, and practice to assure continuous quality of patient care, • Promotes a teamwork approach by offering information, advice, and assistance to all unit staff members in a positive, courteous, and cooperative manner, • Checks email at least weekly and responds appropriately and timely ; uses company-provided email for all work-related email communications, • Maintains regular and predictable attendance and punctuality, • Adheres to all company policies and procedures This job exists in the medical unit of a post-acute facility.
