POSITION RESPONSIBILITIES: Essential Job Functions: Primary Care & Patient Management. The Family Medicine Nurse Practitioner/Physician Assistant (or Family Medicine Provider) will deliver comprehensive primary care to an assigned patient panel in accordance with DHA-PI 6025.11. Serving as the Primary Care Manager (PCM), the provider will be the central point of contact for patients healthcare needs ensuring continuity of care, promoting preventative health, and maintaining the medical readiness of the patient population. The PCM must also be knowledgeable in military-specic healthcare requirements, policies, and procedures. Direct Patient Care: Provide comprehensive medical evaluations, including history and physical examinations, for patients of all ages. Diagnose and treat acute and chronic illnesses, injuries, and other medical conditions. Order and interpret diagnostic tests, such as laboratory studies, X-rays, and other imaging procedures. Develop and implement individualized treatment plans based on patient needs and evidence-based guidelines. Prescribe medications and other therapies, adhering to the Air Force Formulary and established protocols. Perform minor surgical procedures (e.g., wound care, skin biopsies, joint injections) as appropriate and within scope of practice. Provide womens health services, including routine gynecological exams and family planning counseling (if within scope of practice). Provide pediatric care, including well-child exams, immunizations, and treatment of common childhood illnesses (if within scope of practice). Address behavioral health concerns and provide referrals to mental health professionals as needed. Coordinate patient care with specialist and other healthcare providers. Readiness & Military Specic Responsibilities. The provider will support the Air Force mission by maintaining the health and deploy ability of service members through the following duties: Medical Readiness Oversight: Monitor and manage Individual Medical Readiness (IMR) metrics, including immunizations, Preventative Health Assessments (PHA), Mental Health Assessments (MHA), and dental readiness. Health Assessments & Deployment Preparation: Conduct PHAs, pre-deployment screenings, and other readiness evaluations. Deployment Health Management: Diagnose, treat, and follow up on deployment-related conditions and injuries. Operational Medical Support: Provide clinical support during base exercises, contingency operations, and public health emergencies as needed. JOB DESCRIPTION: The MRRCT Day Shift Project Manager is responsible for overseeing a highly motivated and effective crisis response outreach team, representing the roles of Clinical Supervisors, Mental Health Professional Outreach Specialists, Certified Peer Outreach Specialists, and Behavioral Health Outreach Specialists. This team will respond to the crisis needs of community members throughout the central region of King County, as dispatched by Crisis Connections/988, and will provide follow up care or recommendations as deemed clinically appropriate. The Day Shift MRRCT Project Manager will be responsible for overseeing their team’s collaborative and engaging connections with law enforcement, fire/medics, designated community responders, emergency room staff, and crisis connections dispatch team, as well as other community partners and referents. About the Role Charlie Health is hiring exceptional candidates with a Masters degree in mental health and previous work experience facilitating groups to provide remote, telehealth services to our clients. We’re eager to work with forward-thinking mental health professionals to enhance our programming and provide the best possible care to our clients. Responsibilities • Complete all Onboarding requirements within 2 weeks of start date, • Meet with your assigned Charlie Health Group Quality Supervisor or Group Quality Director at a minimum of 1x/month for required check-in, • Respond to all email and Slack communication promptly (within 48 hours of receiving a communication), • Review the curriculum aligned to group assignment prior to group start time, • Arrive ~10 minutes early to scheduled group time and facilitate all groups for the entirety of the hour, • Facilitate groups following Charlie Health best practices and using assigned and most up to date Charlie Health curriculum, • Facilitate groups across age groups and cohorts, including Integrative curriculum, Support Staffing and Wellness Hour as needed, • Foster client engagement and group cohesion throughout the session, encouraging client participation and fostering camera-on culture, • Following all operational policies and procedures as indicated by Charlie Health best practices, • Participate in collaborative Treatment Team (Tuesdays) and Group Supervision (every other Friday) with your assigned Charlie Health Group Quality Supervisor or Group Quality Director, peers and other Clinical Leadership team members, • Work collaboratively and respectfully across the care team including with Primary Therapists, Care Experience Specialists and Care Coaches, • Communicate clearly, professionally, and promptly with all clients, staff, families, agencies, and referents, • Monitor your treatment documentation to ensure all of your notes are completed within required 24 hour timeframe and meet agency and professional standards per DHCS and The Joint Commission standards, • Maintain and model professional ethics, including appropriate boundaries and confidentiality, • Must be available during late afternoons and evenings on weekdays (3-8pm MT on Monday to Thursday) and on Saturdays to meet the schedules of our clients, • Master’s degree in mental health or related field required (see below for full list), • Experience working with a wide range of ages, including children, teens, young adults, and adults clients, • Well versed in a variety of modalities and feel confident integrating them into your treatment approach (DBT, CBT, EMDR, MI certification is a plus), • Passionate about the benefits of group treatment and skilled in conducting group treatment, • Ability to work effectively in a team, • Creative and engaging, especially over video!, • The technical ability to run effective telehealth sessions, • Reliable WIFI connection when meeting with clients, • Familiarity with and willingness to use cloud-based communication software—Gmail, Slack, Zoom, Dropbox–in addition to EMR and outcomes survey software on a daily basis, • Flexibility: Our virtual program allows clinicians the ability to work from home or wherever they are most comfortable., • Support: All of our clinicians receive support from a full time Admissions and Assessment team so that our talented clinicians can focus on what matters most–providing exceptional care to our clients., • Ability to Develop Strong Relationships with Incredible Clients: We may be biased, but we think our clients are incredible.
