Job Description Title: Case Manager Location: Brooklyn, NY Pay Range: $24.37 - $26.37 per hour (effectively $45,000 - $48,000 per year) FLSA Classification: Full-time (35 hours), Non-exempt Role Summary: We are seeking a compassionate and dedicated Case Manager to join our team. The successful candidate will provide a comprehensive range of on-site, person-centered, and strengths-based services to a caseload of approximately 20-25 clients. Primary Job Responsibilities/Duties: The Case Manager's role encompasses, but is not limited to: • Conducting initial intake assessments and promptly addressing critical needs., • Utilizing assessment tools, such as CARES, to identify support service requirements and make immediate referrals., • Orienting clients to the shelter program, its requirements, and potential consequences., • Developing Independent Living Plans (ILPs) in coordination with other Social Service providers, ensuring the delivery of comprehensive support services., • Assisting non-working clients in accessing Public Assistance., • Completing psycho-social assessments within the first 30 days of intake., • Monitoring and tracking client progress., • Empowering clients to actively participate in their own planning and goal setting., • Conducting regular meetings with clients (weekly/bi-weekly or as needed).Referring clients for job development opportunities if appropriate., • Connecting clients with suitable resources to help them achieve their goals., • Meeting with clients to address challenges, successes, and strengthen their capacity for long-term self-sufficiency., • Maintaining thorough client files, documenting conversations, warnings, and progress reports, while preserving confidentiality., • Reporting critical incidents immediately to the Site Director/Director of Social Services., • Collaborating closely with other Housing Specialists and Employment staff to identify client readiness for employment and housing., • Treating all clients, visitors, and employees with kindness, respect, and dignity., • Serving as an advocate for clients when necessary., • Providing periodic case management and counseling, extending one year beyond the client's departure., • Establishing and nurturing connections with other programs and units within the organization to facilitate inter-agency referrals., • Escorting clients to housing viewings, DHS or HRA appointments as required., • Facilitating suitable housing placements for clients., • Ensuring that case notes are completed within 24 hours after client meetings., • Offering support as a substitute for other staff when needed., • Performing general clerical duties. Job Description The senior Case Manager will lead and support shelter case management services that maximize safety, connect residents to needed services and support. The Senior Case Manager supervises Case Managers to ensure standards are met and discusses solutions to resident barriers. Each individual's goals, interests, needs, and skills must be taken into account when developing the schedules. * Solicit feedback, observations, insights, & recommendations from the individual, Rehabilitation Technicians, & Job Developer regularly on individual's progress & changes to make to schedules & Habilitation Plans. * Responsible for submitting & balancing Petty Cash reports, tracking Metro Card distribution & reconciliation, submitting monthly transportation reimbursements, and conducting monthly fire drills at offsite locations. * Visit hub sites & volunteer / internship sites to supervise activities, evaluate individual's progress, and monitor the quality of services/ activities and interactions between Rehabilitations Technicians and individuals. * Responsible for ensuring Rehabilitation Technicians provide daily services as written in the individual's Habilitation Plan and accurately enter time & attendance and services provided to individual's on a daily basis. * In accordance with OPWDD and agency requirements modifies his / her own communication to ensure understanding & respect when interacting with individuals; including modifying tone of voice and use of verbal & nonverbal techniques to meet the individual's communication needs. * Exercises good judgment and discretion in all communications and when making decisions that impact individuals, the group of individuals, and / or in crisis situations. * Demonstrates the following desirable professional qualities: Professional demeanor, follows punctuality & attendance policies, reliability, readily adapts to changes in work assignments, good communication skills, and pleasantness. * For Prevocational Services, develop & support the Individual's skills to help the Individual meet the productivity expectations of the work place. * Develop and maintain positive & productive relationships with coworkers, supervisors, and other colleagues, individuals, parents & caretakers, support teams, and internship / volunteer sites. * Other duties as needed or required. Scope of Responsibility & Positions Supervised: * Supervise Rehabilitation Technicians implementation of schedules & services. Provide support and training on how to positively interact with individuals. Meet with Rehabilitation Technicians and Peers weekly to discuss individuals goals and achievements and coordinate schedules. * Address performance issues with Rehabilitation Technicians including punctuality, absences, communication issues and relate any issues to Supervisor. Create Performance Improvement Plans when necessary.
