Job Description The Shared Medical Appointment (SMA) Coordinator is responsible for the successful coordination of shared medical visits. They are focused on front desk functions as well as coordinating visits and activities for the participants. The SMA Coordinator approaches their tasks in a team-based care fashion that supports patients and their families in self-management, self-efficacy, and behavior change. You can contribute pre-tax or after-tax (Roth) towards your retirement savings., • Flexible Spending Accounts, • Basic Life and AD&D, • Supplemental Life and AD&D, • Short-Term & Long-Term Disability, • Accident & Critical Illness Insurance Specific Duties: • Works in conjunction with other Medical Providers, Dental Providers, Behavioral Health Providers, and all other staff to ensure that the patient obtains the highest standard of integrated health care, • Patient recruitment and education on shared medical visits, • Coordinates physical rooms, staffing, and electronic health record management following standard protocols, • Orders supplies, snacks, and equipment such as binders, marketing materials, and facilitator guides necessary for the efficient operation of the clinic, • Responsible for completing front desk functions, including appointment preparation, patient registration, insurance verification, money collections, patient reminders, etc., • May serve as a greeter, front desk receptionist, and/or cashier when not involved in group-related tasks, • Prepares educational materials and supplies for each cohort/patient meeting, • Communicates regularly with all providers, facilitators, and support staff regarding the SMA visits and program to ensure all are properly informed and answer any questions regarding its coordination, • Provides patients with approved resources and information pertaining to their visits, • Coordinates efforts with the Maternal Child Health Coordinator (MCH Coordinator) for patient referrals, hospital records, and visits for OB patients, • Serve as the expert in job functions in their area. They are the go-to person for questions, support, and staff training, • Assists with data collection and evaluation of the SMA program. Reporting and solving problems that may arise, • Follows procedures for the electronic health records system, • Promotes to all staff a “excellence in customer service” attitude to meet the needs of all SALUD patients, • Ensures safety standards are met in the Front Office as identified by AAAHC, OSHA and Salud Policies/Procedures, • Respects privacy by adhering to all HIPAA standards for patient and employee confidentiality, • Acts as a patient advocate for our vulnerable populations by ensuring that patients are fully informed and comfortable with their visit to SALUD, • Career ladder positions at Salud are required to complete the career ladder through level four, which is most closely associated with their role. This includes completing all the courses, passing the exams, having satisfactory reviews, and obtaining the outside certification as quickly as these levels are available and within the parameters established by Salud. Those unable to complete the ladder within the parameters set by the organization will not be allowed to continue in their current role. May be required to travel to other clinics based on staffing shortages, • Performs other duties as assignedQualifications Education: • Two years of front desk/customer service experience in a medical setting preferred, • Must work well with the other patient care team members, • Ability to communicate clearly and appropriately with diverse patient populations, • Exhibits professionalism, positive attitude, and excellent customer service, • Proficient in elementary math, reading comprehension, and writing skills, • Ability to learn and implement complex policies and procedures, • Familiarity with medical/dental terminology preferred, • Ability to read, understand, and follow verbal and written instructions, • Ability to work evening or weekend hours if needed, • Ability to work under stress, • Ability to recognize, evaluate, solve problems, and correct errors, • Ability to explain and summarize comprehensive concepts in a way that the general population can understand, • Effective organizational, time management, and delegation skills, • Skills in planning, organizing, and coordinating groups, • Ability to take initiative, display good judgment, problem-solving, and decision-making, • Basic knowledge of OB care, from pregnancy testing to prenatal care and postpartum care, • Understanding and willingness to assist with training in both a one-on-one setting and a group setting., • Demonstrates skill and enthusiasm in co-facilitating group visits and is self-motivated, • Must be able to use word processing software, databases, spreadsheets, and eClinicalWorks effectively, • Ability to work with diverse populations Additional Information Salud Family Health is an Equal Opportunity Employer.
