The ideal candidate should be familiar with gravity sanitary main, sanitary force main, pump station and wastewater treatment plant components and operation. Job Description Summary: The Family Team Conference Coordinator coordinates and performs an array of family team conferences (FTCs) involving families enrolled in the Safe Way Forward prevention program. Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients. The SWF program will serve a total of 65 families residing in Brooklyn. POSITION SUMMARY: The Therapeutic Case Manager is responsible for screenings and assessment, engagement, safety planning and coordination of services and treatment for members of the family system experiencing IPV. Approach to service is family-centered, trauma-informed, strengths-based and emphasizes both short-term and long-term goals to restore and maintain Safety for all family members, Protect children from future exposure to IPV, and promote Healing and Improved Family Relationships. This joint prevention-clinical services model offers comprehensive trauma-informed services to the whole family, including the survivor of IPV, the person causing harm (PCH), and to children exposed to the IPV in the home. Range of services includes in-home visitation, crisis intervention, case management, advocacy, individual, group and family counseling, psycho-education and support, and referral to other appropriate services. The SWF program will serve a total of 65 families residing in Brooklyn. POSITION SUMMARY: The Therapeutic Case Manager is responsible for screenings and assessment, engagement, safety planning and coordination of services and treatment for members of the family system experiencing IPV. Approach to service is family-centered, trauma-informed, strengths-based and emphasizes both short-term and long-term goals to restore and maintain Safety for all family members, Protect children from future exposure to IPV, and promote Healing and Improved Family Relationships. The qualified candidate will be responsible for adherence to reporting requirements and timelines and meeting programmatic outcome measures. Job Description Property Manager – Job Description The Property Manager will manage a portfolio of residential co-ops and condos while maintaining the company’s high standard for client service. The ideal candidate is an experienced, hands-on professional responsible for all aspects of property management, including communication with board members, residents, and building staff. Job Description Summary: The Family Team Conference Coordinator coordinates and performs an array of family team conferences (FTCs) involving families enrolled in the Safe Way Forward prevention program. Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients. Essential Job Functions: • Facilitate FTCs for families and staff to review services, assess progress together, and help develop a plan of action to either ensure further progress towards positive child welfare outcomes or end services as appropriate., • Coordinate with program staff and families the scheduling of FTCs and re-scheduling of missed and/or cancelled meetings., • Coordinate & Assign FTC request to ensure the scheduling of separate conferences for SURV & PCH., • Conduct/facilitate FTCs according to ACS protocol, FTC model and maintain fidelity of the FTC model; with the case planning team and families (in homes and virtually)., • Engage participants in non-ACS facilitated Family Team Conferences; which includes reassessment planning conferences and service termination conferences., • Conduct a brief review of prior FTC agreement service plan, family history in CONNECTIONS and hard case record prior to the FTC., • Conduct & participate in debrief meetings prior to FTC and when applicable (a debrief might be required) immediately after an FTC., • Ensure debrief meetings occur consistently with Case Planner, Trauma Counselor and/or unit Clinical Supervisor, to consult about any unresolved questions and/or concerns., • Ensure all conference participants sign Summary and Action Plan following conferences; for virtual conferences, facilitator will include a clause stating, “participants signed electronically”., • Facilitate up to three conferences per day., • Ensure Language Interpretation Services are secured when an FTC request is submitted and identifies a family needs translation., • Responsible for timely documentation of FTC’s Summary Plan and date entry of family team conferences in PROMIS and CONNECTIONS; within 72 hours of FTC., • Float within designated borough to provide FTC facilitation services as needed for the SWF program., • Review and identify external child welfare resources and community linkages., • Use conferences to model effective strategies for engagement, assessment and problem solving when appropriate., • Attend and participate in program staff meetings, case consultations, and related agency, model developer, and external training as appropriate., • Use knowledge of different types of FTCs and EBMs (e.g., high, medium, low) to make referral recommendations., • Work collaboratively with team members on challenging issues to resolve barriers or to develop effective interventions., • Utilize knowledge about ACS standards, SWF internal FTC policies and protocol in helping team to arrive at safety and well-being decisions., • Coordinate Elevated Risk Conferences with ACS.r Qualifications: Utilize the six-stage model for FTCs and encourage and support the use of specific program models (Solution-Based Casework, Functional Family Therapy, and Family Treatment/Rehabilitation) in interactions with families and staff.
