Job Description The pediatrics Nurse will be responsible for the provision of quality home nursing services to clients within the agency's philosophy, policy, and Standards of Community Health Nursing Practice. ESSENTIAL JOB DUTIES: • Initiates, implements, and supervises patient plan of care., • Demonstrate the ability to observe record and report needs and concerns of the client., • Coordinate the needs of home care clients and maintain constructive relationships with them and with all care providers., • Carries out the Plan of Care as established by the RN Case Manager and approved by the physician., • Demonstrates accurate and thorough documentation skills and compliance with documentation guidelines., • Maintains productivity standards as defined by the Director of Patient Services., • Monitor client's progress towards stated goals and communicate with the RN Case Manager as needed for adjustments to the Plan of Care., • Performs medication review, accountability, and administration., • Performs enteral feeding and IV infusions as ordered., • Performs tracheostomy, catheter and ostomy care as ordered., • Evaluates the appropriateness, cleanliness, and safety of durable medical equipment, supplies and/or appliances., • Assists with household tasks directly essential to the patient's health, including planning and preparing nutritious meals, assisting in feeding, light housekeeping of client's immediate environment, changing of bed linens and making bed, and laundering of client's clothing and/or bedding., • Assists the client in learning appropriate self-care techniques., • May perform aide supervision as directed., • Communicates with other members of the health care team., • Adheres to established policies and procedures., • Complete company and state mandated forms and trainings in a timely manner., • Maintain a professional attitude and dress appropriately., • Be responsive to members of Sunshine Home Care when they contact and keep the company apprised of current schedule., • Inform the office of all changes with client, personal health and scheduling., • Pediatric cases - accompany patient to school for 1 on 1 care.
