Essential Job Functions Overview With general supervision of the Care Management Program Manager, works from a trauma informed care perspective to provide support, advocacy, linkage, and coordination of services in a care management program for persons with mental illness and/or chronic health conditions, who qualify for Health Home services as designated by the Department of Health. Individual should strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences. Essential Job Functions: • Provide face to face services, including home visits and telephonic contact on a monthly basis to each individual on their caseload, • Conduct comprehensive assessments to identify an individual’s clinical and psychosocial needs, choices, and preferences for services and to build a person-centered plan of care, • Effectively support individuals through skills and practices including but not limited to motivational interviewing suicide prevention, risk screening, trauma-informed care and person-centered planning., • Responsible for, but not limited to comprehensive assessment, outreach and engagement, service and treatment linkages and coordination using evidence-based practices and outcomes, • Demonstrate proficiency at navigating the health care system, including ability to make referrals to housing services, crisis intervention, peer support., • Support consumers using trauma informed practices with linkages to identified resources, coordination of care among providers, advocacy, and support with identified recovery goals., • Develop and revise individual plans of care consistent with Health Home requirements and coordinating with the Managed Care organizations for HARP members., • Develop and maintain professional relationships through open communication and strong collaboration with community services., • Personally assist consumers with identifying and achieving person centered goals and recovery, • Monitor consumer wellness and ensure well-coordinated care among all providers, • Develop and maintain appropriate and accurate records and files according to all county and organization policies and procedures as well as all governing and regulatory standards, • Attend necessary meetings, • Maintain regular and effective communications with supervisor, county service providers, and all relevant parties as needed, • Collaborate with hospital or treatment providing staff as well as Managed Care Organizations for successful transitions of care, • Address the quality, adequacy, and continuity of services to ensure appropriate support for individuals mental health and psychosocial health needs, • Meet weekly to bi-weekly for supervision, participate case conferences, and other relevant meetings and trainings, • Participate in On-call rotation, • Adhere to Medicaid, Department of Health and Health Homes billing standards, • Secure all health records and other protected information with the highest regard to confidentiality and HIPAA laws and regulations, • Engage families, natural supports, and providers into the care coordination process Education and Experience are dependent on the need of the program at the time of the opening: Care Manager Standard Qualifications: Typical qualifications considered would be a high school diploma and 2 years of relevant experience, or associate’s degree in human services, or related field, plus 1 year of relevant experience, or a bachelor’s degree in a Human Services, or related field. Social Work Case Manager (LMSW/LCSW) - Emergency Department, Maternal Child Health As a member of a multidisciplinary health care team, the Social Work Case Manager provides psychosocial interventions to patients, as well as their families and decision makers, receiving care within the health system and across the care continuum. The Social Work Case Manager conducts psychosocial assessment of patients/families as clinically indicated to evaluate social, emotional, and physical needs by applying social work theories and techniques. (LawNY) has an immediate opening for a Staff Attorney at its office located in Ithaca, NY. The staff attorney will provide legal representation to clients who are trying to overcome barriers to employment caused by their old criminal records. The Reentry Project is a job creation/job retention project, and a key component of LawNYs racial justice efforts. According to the Pew Research Center, in 2018, black Americans represented 33% of the U.S. sentenced prison population, nearly triple their 12% share of the U.S. adult population. By challenging illegal barriers to employment, LawNYs Reentry Project helps clients secure employment licenses, seal old criminal records, and navigate complex legal processes, thereby increasing the clients chances of attaining self-sufficiency. LawNY currently operates a regional pro bono reentry project, with funding from the Legal Services Corporations Pro Bono Innovation Fund (PBIF). The Staff Attorney will provide support to the LawNY pro bono team operating the PBIF project.
