An effective member of this team will communicate well in English and Spanish, understand the policies and regulations around the referral sources, be highly organized, and most importantly be transparent by documenting each task thoroughly. This team member will be solely responsible for the organization of all information pertaining to these patients; they will comply with an organizational protocol supported by each team member.
