Job Description JOB SUMMARY The Case Manager will support and administratively/therapeutically engage the residents progress toward independence, housing, coordination and collaborating services. Perform Independent Living Plans (ILPs), psychosocial and progress notes, documenting resident health, wellbeing and care while in BAVR. Activities with Persons Served • Participate in orientation of new residents by explaining the types of clinical services, recreational activities, and ICL programs that are available to residents., • Obtains and documents or files assessment information, such as medical, psychological, and social factors contributing to the resident's situation as well as information about the residents’ strengths, needs, abilities and preferences., • Collaborates with residents to utilize assessment information to complete and maintain resident service plans, offering an Integrated Healthcare approach to services and ensuring that plans include consideration of the residents’ cultural and ethnic background, customs, needs, beliefs and preferred language., • Instructs the resident in daily living skills, socialization skill enhancement and conflict resolution., • Provides supportive counseling in 1:1 or group formats that assist residents with modifying attitudes and behaviors as needed, by increasing understanding of self and others and by addressing service needs, strengths, abilities and environmental resources to attain goals., • Obtains and develops resource information for residents in all aspects of their care; disseminates this information to residents in a manner that is clear and understandable., • Uses cognitive-behavioral techniques (e.g. role-plays, skills training, plans of action) to assist the resident in meeting service plan goals., • May provide Critical Time Intervention procedures., • Facilitates individualized services to the resident that meet the diverse needs of the resident and focus on the discharge-planning goal., • Performs crisis assessment and nonverbal and verbal crisis intervention., • 1:1 Meet at least weekly with residents to follow-up on service plan goals and objectives., • Refers residents to community resources and other organizations; accompanies residents on regularly scheduled or emergency visits to medical treatment facilities, social agencies, government offices, or other locations associated with the treatment or assistance of the resident., • Provide residents on both a group and an individual basis with transportation for housing placement, service appointments, recreational, or other service plan related purposes as appropriate, • Assist residents with developing a savings and money management plans within the scope of Department of Homeless Services and Veterans Administration regulations and optimizing resident income through advocacy with the HRA and VA; will offer and provide residents both group and individual instruction in budgeting, banking, resident skills such as shopping wisely from a price and product perspective, credit, and taxes., • Plans, escorts, and/or arrange social or recreational activities for and with residents., • Advocates on behalf of resident with outside service providers and within ICL. - Assists residents in finding and getting into vocational training or other training opportunities and work with other team members (e.g. Substance Abuse Specialist) in addressing the needs of residents., • Accompanies resident to appointments as needed. - Gives support & advice to significant others of the residents and Transitional Veterans Residence staff. The aim of Community First is to create an upstream diversion from arrest and justice system involvement by building supportive relationships with people in need of services and creating linkages to mental health services, housing support, substance use services, and other kinds of social services as needed. Community First is a partnership between Midtown Community Justice Center, Breaking Ground, Fountain House, Times Square Alliance, Housing Works, and Ryan Health. Reporting to the Clinical Director, the Community Navigator will be tasked with engaging community members frequenting the Times Square and Midtown Manhattan area who are experiencing housing insecurity or homelessness and often have co-occurring behavioral health challenges. In keeping with the Community First model, the Navigator will build trusting relationships with those individuals, and facilitate linkages to social services and other community-based resources. The Community Navigator will also work with clients of MCJC’s onsite programs, including court-based and diversion programs, who are unstably housed. Community Navigators are peers who share some lived experience with clients and use their own experience in establishing trusted relationships. Job Description Position Summary: Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City’s Asian community and the second largest provider in the country. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. HMH is hiring for a full-time Behavioral Health Clinician responsible for direct clinical work with clients in the outpatient mental health clinic, with a priority focus on those in the CONNECT program. Job Description The Qualified Intellectual Disability Professional / QIDP is responsible to assist and ensure the overall operations and services are being delivered according to state and federal regulations, agency policy and procedure and according to the agency’s mission. The role of the Qualified Intellectual Disability Professional / QIDP is responsible to assist in the overall coordinating and managing the overall person-centered services for assigned individuals. The Qualified Individual Disability Professional (QIDP) serves to provide leadership, direction, and training in order to supervise the overall development of a specific program. QIDP also supervises the rehabilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, develop implementation strategies which outline services and supports for individuals so they may work towards the outcomes detailed in their Personal Plan. The QIDP meets routinely with other Team Clinicians for coordination and integration of services. The QIDP coordinates with the Residential Manager(s) and staff.
