Job Description In the context of child-centered, the Case Planner provides family-focused practice to promote permanency, safety and well-being, and to reduce length of stay in foster care, the case planner also provides comprehensive casework services for both youth and families. The case planner provides all case management services and ensures that all ACS mandates are in compliance. Job Summary: The Case Planner provides comprehensive case management services for all youth who reside in their respective Site i.e. Non-Secure Placement or Limited-Secure Placement facility and their families. The Case Planner should be committed to a service philosophy and objectives that is youth-centered and outcome focused. The position is demanding and highly interactive with the youth and their families, staff and community. Case Planners must be comfortable working with youth and families in the context of the facility, the office and in the community of origin. The Case Planner will maintain a caseload of 15 youth both in the facility and in the community on aftercare. The role of the Case Planner is to streamline services and service plan goals, guide the discharge readiness planning beginning once youth are placed, and follow through with the youth/family in the application of the service plan in the community. Job DescriptionJob Summary The Case Planner Supervisor is responsible for the supervision of Case Planners in an Enhanced family foster care program. S/he will play an integral role with assisting Case Planners with the integration of trauma principles into case planning activities. This outreach prioritizes referrals from partner schools, encompassing educators, support staff, and school-based service providers (health and mental health clinics, after-school programs, community school lead organizations, etc.), and extends to the broader community-based organizations (CBOs) and internal CAMBA programs to ensure a holistic approach to family support., • Collaborates with referral sources, including schools (educators, support staff, and school-based service providers) and CBOs, to gain a comprehensive understanding of each family's unique strengths, needs, and priorities, ensuring a good program fit. In this collaboration, informed consent for information sharing is secured from families to facilitate service delivery., • Develops individualized support plans in partnership with families.
