As a Dietitian at U.S. Renal Care, you will be an integral part of an interdisciplinary team, providing excellent care for patients living with kidney disease. U.S. Renal care dietitians work under the supervision of a RDN Regional Manager. Patient Nutrition. Using the most current, science-based practices, you will recommend therapeutic diets based on patient nutritional needs, preferences, and changes in treatment, in consultation with the patient's physician. Position Summary: Primarily responsible for providing intensive home, school, and community-based Mental Health Rehabilitation services to children/adolescents and adults. Performs clinical assessment, brief psychotherapy, consultation, and linkage to other services for clients and/or their families. This is a full-time position and requires working flexible hours according to the needs of our clients (which includes some evenings and weekends). This position provides a variety of specialized services to special populations. The CHW is expected to have an understanding of mental health conditions and addiction and be willing and ready to engage with clients who struggle with these issues at all levels. The CHW is responsible for promoting behavior changes, assisting with linkages to appropriate health care services and assisting individuals with self-management of chronic diseases.
