    SCOPE OF ROLE: The Case Manager provides and coordinates client services. II. ESSENTIAL DUTIES & RESPONSIBILITIES: • Conduct intake process with new clients within 24 hours of admission; includes but is not limited to explanation of resident rights and responsibilities, and grievance procedures • Conduct psychosocial, vocational/educational, and housing assessments and documentation of same • Provide accurate, complete, and timely documentation of the provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing • Prepare housing applications and assist clients with the housing application/interview process., • Monitor and assess client progression/regression in behavior and/or appearance, • Facilitate group and individual counseling, workshops, psych education, and recreational activities, • Conduct room/dorm visits/inspections to assess client personal hygiene, room cleanliness, and overall, ADL skills • Provide comprehensive case management services to an assigned caseload, • Develop initial service plans and conduct service plan reviews as needed for an assigned caseload, • Provide referrals for in-house and community-based medical, mental health, and supportive social services to assigned caseload as appropriate • Ensure that clients attend meals and maintain appropriate nutritional intake, • Provide crisis intervention and address all emergency situations as appropriate, • Assist in building evacuation during fire drills and building emergencies, • Monitor client medication compliance and collaborate with nursing staff monitor compliance, • Refer clients to appropriate community resources as needed, • Maintain CPR/First Aid Certification, • Maintain up to date, accurate, complete, and timely client chart documentation in compliance with DHS, OTDA, other Funder and SUS standards • Facilitate discharge/transfer process and documentation of same JOB DESCRIPTION • Attend staff meetings, case conferences, group supervision, training, and escort clients on housing interviews as needed • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes • A work schedule that includes some weekend and evening hours is required, • Other duties as assigned III. EOE POSITION SUMMARY: CRED is a cohort-based vocational training program for at-risk individuals between the ages of 18-40 years old. The program provides classroom instruction, case management services, internship opportunities, and job placement services designed to help clients earn a Commercial Driver’s License and obtain employment in the transportation industry. The Social Worker will be responsible for providing comprehensive case management services including, but not limited to, mental health support, career counseling, personal guidance and development, life skills, and referrals. The Social Worker will work in both individual and group settings to help clients overcome personal and professional barriers. The individual selected for this position will be under the direct supervision of the CRED Program Director, will conduct themselves in a professional manner, and will comply with all standards set forth by United Activities Unlimited and the NYC Department of Youth & Community Development. PRIMARY JOB RESPONSIBILITIES (include but are not limited to): • Conduct intake interviews to understand each client’s individual needs, strengths, and goals., • Collect all required program documentation from clients, including enrollment, outcome, and follow-up paperwork., • Work with clients to develop an Individual Service Strategy (ISS) that reflects SMART short-and-long-term goals., • Provide case management services including regular check-ins to assess progress towards personal and professional goals., • Provide individual and group counseling, including but not limited to, mental health, behavior management, and mediation services., • Work collaboratively with other CRED staff members to conduct case planning and draft case notes according to DYCD requirements., • Refer clients in need of greater support to community based social service providers., • Maintain positive working relationships with social services agencies and other community partners to enrich programming, internship, and employment opportunities in alignment with client’s interests and ISS., • Assist other CRED staff members with conducting follow-up services, including obtaining compliance documentation from former clients., • Collect, maintain, and audit client files for completion and accuracy in accordance with DYCD and UAU policies., • Participate in team meetings, conferences, professional development trainings, and other program-related events and activities., • Work collaboratively with other CRED staff members to ensure the program meets all DYCD goals, deliverables, and deadlines., • Assist with other departmental duties and projects as needed. Evening and Weekend Hours may be required. (Including Weekly Deliveries need to be Stored, Label, and Dated also must be able to lift 25lbs.), • Responsible for the general supervision of kitchen area, including overall cleanliness, maintenance of equipment, utensils, and supplies, and the ability to provide instruction to inexperienced trainees., • Assist in the implementation of a Daily Cooking and & Nutrition (Daily Living Skills) training curriculum for Shelter Residents to prepare them for independent living, • Cook and serve 3 meals per day to 200 shelter residents according to the census and the menu., • Attend staff meetings, conferences, group supervision, and training as needed., • Kitchen cleanliness, and equipment maintenance according to NYC and NYS - DOH guidelines and program requirements., • Prepare and serve meals at SUS Multiple Sites if necessary., • A work schedule that includes some weekend and evening hours will be required., • Comply with and enforce all SUS Corporate Compliance and Employee Policies, Procedures, and Protocols., • Comply with, enforce, and carry out all SUS Incident Management Policies, Procedures, and Protocols as appropriate.
