The Clinical Therapist I will work with a multidisciplinary treatment team to provide psychological, social, and interventions that support patients and families relating to behavioral/medical health, levels of care, disease and illness and wellness and recovery. Responsibilties: A day in the life of an Clinical Therapist I at Hackensack Meridian Health includes: • Provides a comprehensive assessment of a patient's psychosocial needs under supervision of a licensed leader/clinical supervisor and develops treatment plans and treatment plan reviews based on findings and according to department policy., • Provides effective behavioral/medical health treatment and education to patients, including assessment, psychotherapeutic counseling, or therapy (individual and/or group), and shares and interprets information to staff, families, and/or other approved agencies., • Completes all documentation in accordance with department and hospital policy., • Provides all services and interventions to patients in accordance with Licensure regulations defined by the governing licensing board., • Utilizes and obtains and interprets information in terms of patient needs, knowledge of growth, development, and an understanding of the evidence- based range of treatment options for specific populations., • Provides comprehensive case management services/assessment as warranted., • Assess Social Determinants of Health (SDOH) and provide information, education and/or referrals as appropriate to meet identified patient needs., • Collaborates with the patient (with their consent), family, and other support persons involved in the patient's care to develop a comprehensive discharge plan that best meets the continuing care needs of the patient., • Reassesses treatment plan on an ongoing basis , evaluates progress, revises plan when clinically indicated., • Maintains protected patient health information as required by law, State Licensing, TJC and HMH policy., • Assists other behavioral/medical team members in responding to emergencies., • Has a working knowledge of various community agencies and resources and refers patients when appropriate., • Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs. and stands a minimum of 4 hours a day., • Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.) This process includes communicating and providing instruction/education with the patient, parent and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.)., • Adheres to the standards identified in the Medical Center's Organizational Competencies., • Receive proper supervision as per their Licensed Board., • Master's degree in Social Work (MSW), Master's degree in Counseling,or related fields, • 0-3 years of total work experience in Social Work or related fields, • Work under the supervision or guidance of a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) with approved clinical supervision or ACS credential., • Ability to perform psychosocial assessments and develop treatment plans., • Knowledge of crisis intervention theory and its application in a variety of settings., • Knowledge of disease management and disease process and differential diagnosis and treatment techniques., • Ability to work as part of a team, while building collaborative relationships., • Must possess excellent written and verbal communication skills.

Moriviv Residential Program is a 51 bed scattered site safe dwelling. The goal of this program is to provide short term, emergency shelter for people fleeing domestic violence. Program staff work with each participant to develop an individualized action plan that connects them to support services and helps them secure permanent housing. Supportive services include: counseling, support groups, case management, childrens services, referrals for educational/vocational facilities, job preparation and health promotion for women and children. The Program Manager is responsible for the overall daily operations of the program and the safe dwellings. In addition to acting as VIPs primary coordinator to assure that all efforts are fully met and exceed contractual obligations (programmatic and administrative) and direct the day-to-day activities of risk management, safety/security, maintenance and client move-in/turn-over. The Program Manager is responsible for ensuring 24/7 services to all program participants and may also be called upon for late night and weekend assistance. Job Description Astor’s Prevention Services/Family Connections is a time-limited, evidence-based model that utilizes a home-based approach to address both the concrete and clinical needs of the family. Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family. Collaborate with the Housing Coordinators to ensure clients are compliant in seeking permanent housing. · Attend shelter meetings, agency-wide meetings, and staff training (onsite and offsite) as directed by the supervisor. · Must work evenings, one Saturday each month, and holidays as scheduled. · Prepare all mandated reporting as the supervisor, contract, and funding agencies require. · Conduct weekly client unit inspections and review findings with the client and supervisor. · Perform other related duties as needed. Job Description Summary: Case Managers provide comprehensive services to individual clients and families residing in cluster apartments. Services provided by Case Managers include assessment, case planning, developing an Independent Living Plan, case facilitation and management, counseling, referrals, information workshops and support groups. The Case Managers coordinates with other program staff and local agencies to provide short and long term care necessary to meet the needs related to moving families to permanent housing. Essential Job Functions: The Case Manager is responsible for managing a caseload of families and children residing in the shelter. Provides counseling intervention services to residents and coordinates all client services as identified in the Independent Living Plan (ILP).
