Job Description Hourly Rate: $21.92 POSITION SUMMARY: Under the supervision of a Community Supervisor and/or a Lead Direct Support Professional, the Habilitation Counselor is responsible for providing habilitation supports in small group settings to people in varied environments, primarily the home and the community. This person is responsible for implementing programs as well as providing care to individuals. The Habilitation Counselor will provide a normalized, therapeutic, safe environment in the least restrictive atmosphere possible. The Habilitation Counselor promotes the positive self-esteem of individuals through consistent positive interactions, support, encouragement and respect of individual rights. The Habilitation Counselor provides instructional services to people receiving supports based on their interests and desires; and assists individuals in achieving individualized valued outcomes to increase skill development. The Habilitation Counselor documents the delivery of the habilitation activities on a daily basis, generates monthly summary notes and reviews required documentation to ensure compliance with OPWDD regulations and Agency policy.
